ImportanceThisstudyrepresentsthemostrecentepidemiologicaltrendsofheadandneckcancerintheUnitedstates.ItprovidesimportantdiscussionwithregardstooropharyngealandHPVrelatedcancers.ObjectiveToidentifytrendsinheadandneckcancer(2002-2012)withintheUnitedStates.StudyDesignRetrospectiveanalysisoftheUnitedStatesNationalCancerInstitute'sSurveillance,Epidemiology,andEnd-Results(SEER)MethodsUsingtheNovember2014submissionoftheSEERdatabaseandSEER-18datafiles,datafrom2002-2012wasanalyzedtodeterminethemostrecentepidemiologicaltrends.Headandneckcancers(HNC)ofallsubtypeswereanalyzedtogether.Laryngealcancerswerefurtheranalyzedseparately.Finally,oropharyngealcancersofthebaseoftongueandtonsilwereanalyzedindependentlytoattempttotrendHPVrelatedcancers.ResultsFrom2002to2012,therewere149,301casesofHNCrecordedintheSEERDatabase.TheHNCratedecreasedby0.22%peryear(p=0.0549),andtherateoflaryngealcancerdecreasedby1.9%peryear(p<0.0001).Therateoforopharyngeal(HPVrelated)increasedby2.5%peryear(p<0.0001).Finally,HNCrateswerefoundtobesignificantlyincreasinginKentuckyandConnecticut,anddecreasinginCalifornia(p<0.05).HPVrelatedcancersaresignificantlyincreasinginallstatesexceptGeorgia,Hawaii,andMichigan(p<0.05).LaryngealcancerratesaredecreasinginCalifornia,Georgia,NewJerseyandNewMexico(p<0.05).ConclusionsTheoverallincidenceofHNCisdecreasingintheUnitedStates.ThereisarisingincidenceofHPVrelatedcancersoftheoropharynx.Significantdifferencesincancerincidenceandrateofchangeexistbetweenmalesandfemales.Furthermore,youngeragegroupshaveamoresignificantdeclineofoverallheadandneckcancer,withanoverallincreaseinHPVrelatedcancerinpatientsolderthan50.
