Tumor-derived factors modulating dendritic cell function
Tumor-derived factors modulating dendritic cell function: AbstractDendritic cells (DC) play unique and diverse roles in the tumor occurrence, development, progression and response to therapy. First of all, DC can actively uptake tumor-associated antigens, process them and present antigenic peptides to T cells inducing and maintaining tumor-specific T cell responses. DC interaction with different immune effector cells may also support innate antitumor immunity, as well as humoral responses also known to inhibit tumor development in certain cases. On the other hand, DC are recruited ... περισσότερα »

Clinical implications of mucosal remodeling from chronic rhinosinusitis
Clinical implications of mucosal remodeling from chronic rhinosinusitis: Background Prognostic implications of mucosal remodeling in chronic rhinosinusitis (CRS) remain unclear. Remodeling of respiratory mucosa in asthma is associated with greater medication use and decreased function. This study investigates the implications of mucosal remodeling on long-term clinical outcomes in patients with CRS. Methods A case-control study of adult patients with CRS undergoing endoscopic sinus surgery (ESS) was performed. Mucosal remodeling was defined by squamous metaplasia, subepithelial fibr...περισσότερα »
Evaluation of bacterial transmission to the paranasal sinuses through sinus irrigation
Evaluation of bacterial transmission to the paranasal sinuses through sinus irrigation: Background Saline nasal irrigation is effective in the treatment of sinonasal disorders, including chronic rhinosinusitis (CRS). Despite bacterial contamination in rinse bottles and reports of infections from contaminated irrigation water, tap water is still used by ∼50% of irrigation users, raising a potential public health concern. This study aimed to determine whether bacteria from the water supply used in sinus irrigations colonizes the paranasal sinuses. Methods Samples were taken from the:... περισσότερα »

Interleukin-9 and interleukin-17C in chronic rhinosinusitis
Interleukin-9 and interleukin-17C in chronic rhinosinusitis: Background Interleukin (IL)-9 and IL-17C have been known to play a role in allergic inflammation, yet, their roles in chronic rhinosinusitis (CRS) are not well defined. IL-9 induces changes in epithelial cell gene expression leading to goblet cell metaplasia, whereas IL-17C is functionally distinct in that its expression can be induced by bacterial challenge and inflammatory stimuli. This study aimed to compare levels of IL-9 and IL-17C in CRS with nasal polyps (CRSwNP) and without nasal polyps (CRSsNP) as well as atopy. M... περισσότερα »

Mouth breathing may bypass the protective function of the nose and is anecdotally considered to increase asthma morbidity. However, there is no epidemiological evidence that mouth breathing is independently associated with asthma morbidity and sensitization to allergens. The choice of double-lumen tube size
How clinical experience leads anesthetists in the choice of double-lumen tube size: Lung separation techniques are used to provide 1-lung ventilation in patients undergoing thoracic, mediastinal, cardiac, vascular, and esophageal procedures [1]. Left-sided double-lumen tubes (DLTs) are the most common devices used for lung isolation because of their greater margin of safety [2]. However, the use of an incorrect DLT size may lead to iatrogenic airway injury or failure to provide proper lung isolation. Noise Cancelling Headphones: Tinnitus is a painful condition that causes people to experience a nearly constant ringing sound in their ears. Infections and certain medical conditions may cause tinnitus to develop, but the most common reason why people get tinnitus is exposure to loud sounds. The more frequently a person is exposed to loud sounds, the more likely they are to develop tinnitus. Noise cancelling headphones for tinnitus use a built-in microphone and electronic circuitry to create a competing noise that cancels out the sound of ringing. There are many benefits to using t... περισσότερα »

Intranasal fentanyl for the prehospital management of acute pain in children.
Introduction: Acute pain is the most common symptom in the emergency setting and its optimal management continues to challenge prehospital emergency care practitioners, particularly in the paediatric population. Difficulty in establishing vascular access and fear of opiate administration to small children are recognized reasons for oligoanalgesia. Intranasal fentanyl (INF) has been shown to be as safe and effective as intravenous morphine in the treatment of severe pain in children in the Emergency Department setting. Aim: This study aimed to describe the clinical efficacy and safet... περισσότερα »
Large arterial occlusive strokes as a medical emergency: need to accurately predict clot location.
Background: Endovascular treatment for acute ischemic stroke with a large intracranial occlusion was recently shown to be effective. Timely knowledge of the presence, site, and extent of arterial occlusions in the ischemic territory has the potential to influence patient selection for endovascular treatment. We aimed to find predictors of large vessel occlusive strokes, on the basis of available demographic, clinical, radiological, and laboratory data in the emergency setting. Methods: Patients enrolled in ASTRAL registry with acute ischemic stroke and computed tomography (CT)-angio... περισσότερα »

Resting arterial hypoxemia in subjects with chronic heart failure, pulmonary hypertension and patent foramen ovale
AlexAndros SfakianakisστοMedicine by Alexandros G.Sfakianakis,Anapafseos 5 Agios Nikolaos,Crete 72100,Greece,tel :00302841026Πριν από 1 ώρα
The roles of intrapulmonary and intracardiac shunt in contributing to arterial hypoxemia at rest in chronic heart failure (CHF) subjects have not been well investigated. We hypothesized that blood flow through intrapulmonary arteriovenous anastomoses (QIPAVA) and/or patent foramen ovale (QPFO) could potentially contribute to arterial hypoxemia and, with pulmonary hypertension (PH) secondary to CHF, this contribution may be exacerbated. Fifty-six CHF subjects (New York Heart Association Classes I-III), with (+) and without (−) PH defined as peak tricuspid regurgitation velocity (T... περισσότερα »

Intercostal artery (ICA) reimplantation (ICAR) is thought to decrease spinal cord injury (SCI) in thoracic aortic aneurysm and thoracoabdominal aortic aneurysm (TAAA) surgery. Patients treated from 1989 to 2005 without ICAR were compared with those treated from 2005 to 2013 with ICAR to determine whether ICAR reduced SCI. We hypothesized that ICAR would reduce SCI, […]

The differential for PET-positive cutaneous lesions includes primary skin cancers, infections, cutaneous metastases from distant malignancies, and benign neoplasms. In dermatology, PET/CT scans have been most widely studied in patients with melanoma and Merkel cell carcinoma. The role of PET/CT scans in the management of other cutaneous malignancies is less clear, but it has shown great promise in the management of patients with squamou... περισσότερα »
