Familial gastrointestinal stromal tumors, lentigines, and café-au-lait macules associated with germline c-kit mutation treated with imatinib

Abstract

Background

Familial lentiginosis syndromes are characterized by a wide array of manifestations resulting from activation of molecular pathways which control growth, proliferation, and differentiation of a broad range of tissues. Familial gastrointestinal stromal tumors (GISTs) are often accompanied by additional features like hyperpigmentation, mastocytosis, and dysphagia. They have been described with mutations in c-kit (most commonly), platelet-derived growth factor receptor A, neurofibromatosis-1, and succinate dehydrogenase genes.

Materials and Methods

We report on molecular characterization and tumor histopathology of two siblings in whom lentigines and café-au-lait macules were present along with multifocal GIST. Immuhistochemical analysis of CD34 and CD117 was performed on GIST biopsy samples from both siblings, while c-kit mutational analysis was done by PCR and direct sequencing on DNA from peripheral blood leukocytes of all family members and from paraffin-embedded gastric biopsy specimens of affected siblings.

Results

Histopathology revealed positive expression of CD117 and CD34. Mutational analysis showed the germline c.1676T>C mutation in c-kit exon 11, (p.(Val559Ala)), in the peripheral blood of both siblings and a second exon 11 mutation, c.1669T>A (p.(Trp557Arg)) in the tumor biopsy of one of them. Initiation of imatinib treatment resulted in striking resolution of their hyperpigmentation and a stable gastrointestinal disease in one of them.

Conclusions

A c-kit mutational test in familial GISTs is indicated before initiation of imatinib therapy, as it can help predict tumor response to treatment.

Protective effects of tacalcitol against oxidative damage in human epidermal melanocytes

Abstract

Background

Oxidative damage may lead to the dysfunction of melanocytes (MCs) and is one of the causative mechanisms involved in the pathogenesis of vitiligo.

Objectives

This study was designed to investigate the protective effects of the vitamin D3 analog tacalcitol on oxidative damage induced by hydrogen peroxide (H2O2) in human epidermal MCs.

Methods

Human epidermal MCs were cultured and identified by l-DOPA staining and HMB-45 immunohistochemical staining. The model of oxidative damage induced by H2O2 was established, and the cells were treated with tacalcitol. The viability of MCs was determined using an MTS assay. Morphological changes in cell dendrites were observed by microscopy, and the rate of change of dendrites was calculated. The reactive oxygen species (ROS) level in MCs was determined using immunofluorescence microscopy. Superoxide dismutase (SOD) activity and malondialdehyde (MDA) levels in MCs were determined using the WST-1 and TBA methods, respectively.

Results

In comparison with the control group, the viability of MCs and SOD activity were significantly decreased in the H2O2 group (P < 0.05) and significantly increased in the tacalcitol group (P < 0.05). In comparison with the control group, the rate of change of cell dendrites and levels of ROS and MDA were significantly increased in the H2O2 group (P < 0.05) and significantly decreased in the tacalcitol group (P < 0.05).

Conclusions

Tacalcitol can reduce oxidative damage induced by H2O2 in MCs by inhibiting intracellular ROS overproduction, increasing SOD activity, and decreasing the level of MDA, thereby reducing cell apoptosis.

Risk factors for nonmelanoma skin cancer in renal transplant recipients: a case–control study from a reference outpatient clinic in Southeast Brazil

Abstract

Background

Nonmelanoma skin cancer (NMSC) is the most common tumor in humans, and its incidence increases among renal transplant recipients (RTRs). The aims of this study were to characterize the RTRs with NMSC, to identify risk factors, and to calculate the probability of this tumor in this population.

Methods

This was a hospital-based case–control study. Epidemiological and clinical variables were evaluated. Hierarchical logistic regression was used, and a mathematical model was built.

Results

In total, 245 subjects were included. Possible associations were identified using a univariate analysis. Multivariate analysis identified risk factors with respective odds ratios and confidence intervals (95% CI): males 2.5 (1.3–4.7), age over 50 years 5.4 (2.3–12.9), Fitzpatrick's skin phototypes I–III 3.7 (1.6–8.7), occupational sun exposure 4.1 (2.1–8.1), timetable of recreational sun exposure all day 3.0 (1.4–6.1), and duration of transplantation (80 months or more) 3.3 (1.6–6.5). The Hosmer-Lemeshow test and the receiver operating characteristics curve showed a strong fit and accuracy, respectively. The probability of an NMSC ranged from less than 1 to 92.5%.

Conclusions

This study characterized the RTRs with NMSC and identified risk factors. The multivariate analysis by hierarchical logistic regression proved to be a useful tool and allowed for the determination of the probability of NMSC in this population.

Trichoscopic clues for diagnosis of alopecia areata and trichotillomania in Asians

Abstract

Background

Trichoscopy has become a useful diagnostic tool for various hair and scalp diseases, including alopecia areata (AA) and trichotillomania (TTM), which are sometimes difficult to distinguish clinically.

Objectives

To describe trichoscopic findings of AA and TTM in an Asian population and to establish diagnostic clues for these conditions.

Methods

Trichoscopy was performed with a handheld dermoscope in 52 patients diagnosed with AA and 23 patients diagnosed with TTM. Trichoscopic images were then blindly evaluated.

Results

The trichoscopic features more frequently observed in AA than in TTM included exclamation mark hairs (AA 59.6%, TTM 26.1%), tapered hairs (AA 59.6%, TTM 4.3%), yellow dots (AA 46.2%, TTM 21.7%), and angulated hairs (AA 26.9%, TTM 0%) (< 0.05). On the other hand, broken hairs of different lengths (TTM 100%, AA 3.8%), trichoptilosis (TTM 78.3%, AA 5.8%), V-sign (TTM 43.5%, AA 3.8%), flame hairs (TTM 43.5%, AA 0%), and hair powder (TTM 13%, AA 1.9%) were more commonly demonstrated in TTM than in AA (< 0.05).

Conclusions

Exclamation mark hairs indicate a diagnosis of AA but not pathognomonic. In addition, angulated hairs, fractured hairs forming a sharp angle along the hair shaft, appear to be typical for AA in Asians when differentiating from TTM. It is important to consider various trichoscopic findings together to establish the diagnosis of AA or TTM.

Efficacy of superficial cryotherapy on the eyebrows of patients with alopecia universalis also treated with contact immunotherapy on the scalp: a prospective, split-face comparative study

Abstract

Background

Few treatment modalities are available for treating alopecia areata (AA) of the eyebrow. Due to the anatomical proximity of the eyebrows to the eyes, safety issues and side effects should always be taken into consideration when choosing the treatment modality. This study was designed to examine the efficacy of superficial cryotherapy on patients with AA of the eyebrow.

Methods

Superficial cryotherapy was performed every other week on the right eyebrow (SC-treated) in a total of 20 patients who had been previously treated with diphenylcyclopropenone (DPCP) immunotherapy on the scalp. No specific treatment was performed on the left eyebrows as a control. The degree of eyebrow recovery was compared in 15 patients who continued to receive more than 10 superficial cryotherapy treatments (5 months of treatment) on their right eyebrow.

Results

Hair density was significantly increased on both treated and control eyebrows after 5 months of treatment compared with the pretreatment density; moreover, the SC-treated eyebrows exhibited a significantly greater increase in density than the control eyebrows. Although hair thickness in the control eyebrows did not change significantly over the treatment period, hair thickness of the SC-treated eyebrows showed a statistically significant increase at months 3 and 5.

Conclusions

Superficial cryotherapy is associated with minimal to no adverse events and exhibits high compliance and relatively good efficacy. Thus, this treatment is an important additional option for patients with AA of the eyebrow.

Clinical effectiveness of low-level laser treatment on peripheral somatosensory neuropathy

Abstract

Peripheral sensory neuropathy treatment is one of the common treatment problems and causes morbidity and mortality in people suffering from that. Although treatment depends on the underlying cause of the condition, nevertheless, in some cases, there is no cure for it, and it requires palliative and symptomatic treatment. In laboratory studies, low-level laser has been effective in the nerves protection and restoration. The aim of this article is to investigate the clinical efficacy of low-level laser on improvement of the peripheral somatosensory neuropathy. Search in the articles published up to 30 October 2015 (full text and abstracts) in databases PubMed (Medline), Cochrane library, Physiotherapy Evidence Database was performed. The studies of low-level laser trials on patients with peripheral neuropathy were carried out and evaluated in terms of the exclusion criteria. There are 35 articles among which 10 articles had the intended and required criteria. 1, 3, and 6 articles study the patients with diabetes, neuropathy caused by trauma, and carpal tunnel syndrome, respectively. In six studies, laser led to a reduction in sensory impairment and improvement of the physiological function of the sensory nerves. In these articles, lasers (Diode, GaAlAs, He-Ne) had wavelength range 660–860 nm, radiation power 20–250 mW, energy density 0.45–70 J/cm2. The intervention sessions range was 6–21 times and patient follow-up was 0–6 months. According to the results of these studies, low-level laser therapy can improve sensory function in patients with peripheral somatosensory neuropathy, although little research have not been done, laser treatment regimens are varied and do not recommend a specific treatment protocol. It seems it requires more research to sum up better, particularly in relation to diabetes.

Assessment of the effectiveness of topical propranolol 4% gel for infantile hemangiomas

Abstract

Background

Infantile hemangiomas (IHs) are the most common vascular tumors in children. Because of their benign character and natural involution, the vast majority of IHs do not require any treatment. In the past few years, topical beta blockers have been reported to be an effective treatment of superficial IHs.

Objective

We sought to evaluate the clinical effectiveness and safety profile of topical propranolol 4% gel for the treatment of IHs.

Methods

A retrospective study of all cases of IHs treated with topical propranolol 4% gel between 2013 and 2015 was performed. All patients were evaluated in a pediatric dermatology unit of a tertiary medical center. Epidemiologic, clinical, and treatment data, including effectiveness score and safety, were reviewed.

Results

The study included 63 patients with a total of 75 IHs. Of the total number of IHs, 43 (57.3%) showed a good response to treatment, 19 (25.3%) a partial response, and 13 (17.33%) poor or no response, thus 62 (82.6%) had good or partial response to treatment. Age at treatment initiation, treatment time, thickness of the superficial component, and size of the lesions were shown to predict response to therapy. Out of the entire examined group, only two patients reported minor local side effects manifested by irritation, redness, and scaling of the treated area. No systemic adverse effects were reported. Limitations: This is an uncontrolled retrospective study.

Conclusion

Propranolol 4% gel is a safe and efficient topical therapy for IH.

In stage pT1 non-muscle-invasive bladder cancer (NMIBC), high KRT20 and low KRT5 mRNA expression identify the luminal subtype and predict recurrence and survival

Abstract

Differential expression of cytokeratins (CK) is a characteristic feature of chemoresistant luminal (KRT20) and chemosensitive intrinsic aggressive basal (KRT5) subtypes in muscle-invasive bladder cancer (MIBC). We investigated mRNA expression of KRT5 and KRT20 and its predictive value in stage pT1 bladder cancer. In retrospective analysis of clinical data and formalin-fixed paraffin-embedded tissues (FFPE) of patients with stage pT1 NMIBC who underwent transurethral resection of the bladder, a single-step RT-qPCR was used to measure mRNA expression. Furthermore, immunohistochemical (IHC) staining of CK20, panCK, and MIB1 was performed. Valid measurements were obtained from 231 samples out of a series of 284 patients. Spearman correlation revealed significant associations between mRNA and protein expression of KRT20/CK20 (ρ 0.6096, p < 0.0001) and MKI67/MIB1 (ρ 0.5467, p < 0.0001). A positive correlation was found between MKI67 and KRT20 expression (ρ 0.3492, p < 0.0001), while MKI67 and KRT5 were negatively correlated (ρ −0.1693, p = 0.01). High KRT20 expression (≥40.26) was significantly associated with worse recurrence free survival (RFS) (p = 0.001), progression-free survival (PFS) (p = 0.0003), and cancer specific survival (CSS) (p = 0.0414). The combination of high KRT20 expression and low KRT5 expression (<36.83) was associated with unfavorable RFS (p = 0.0038) and PFS (p = 0.0003) and proved to be the only independent predictor for RFS (p = 0.0055) and PFS (p = 0.0023) in multivariate analysis. KRT20 mRNA determination was superior to CK20 protein estimation with regard to RFS and PFS prediction. KRT20 and KRT5 mRNA quantification can predict recurrence and progression of stage pT1 NMIBC reflecting basal and luminal subtypes of MIBC and is superior to CK20 protein expression determined by IHC.

ObjectiveThe aim of the current study is to evaluate audiologic and radiologic findings of cochlear hypoplasia which is a subgroup of inner ear malformations.MethodsThis study was a prospective clinical study and based on voluntary participation from cases with cochlear hypoplasia diagnosis. The study was conducted at Hacettepe University, Department of Otolaryngology, Head and Neck Surgery and Department of Audiology. Subjects were selected from an inner ear malformations database. Inclusion criteria were having cochlear hypoplasia for at least one ear. There were 66 subjects with an age range of 12 months and 60 years 5 months. For each subject, pure tone audiometry and tympanometry were applied according to chronological and cognitive age. And also, auditory brainstem response test was applied to when it is need. Subjects' radiologic results were reevaluated to confirm cochlear hypoplasia, cochlear nerve and cochlear aperture.ResultsCochlear hypoplasia types were statistically significantly different in terms of HL degree. This difference was caused by cochlear hypoplasia type IV group being was statistically different from the other three groups. Like with degree of HL, cochlear hypoplasia groups were statistically different from other three groups in terms of type of hearing loss. Cochlear aperture and cochlear nerve status showed variation according to cochlear hypoplasia type but these differences were not statistically approved.ConclusionsIn the current study, incidence of cochlear hypoplasia was 23.5% in all inner ear malformation. With this study, it was seen that subtypes of cochlear hypoplasia showed variability in terms of degree and type of hearing loss and also cochlear aperture and cochlear nerve status. Especially cochlear hypoplasia type IV differs from other three cochlear hypoplasia types.

Our aim was to investigate the differences in the dynamics of lip force between a group of participants with repaired cleft lips and a group of patients the same age but without clefts.We evaluated 101 children between the ages of 10 to 15 years (unilateral clefts n=35, bilateral clefts n=15, class I malocclusion n=25, and class III malocclusion n=26). Introduction: Midface reconstruction remains a challenging area for most craniofacial surgeons. Free tissue transfer has been suggested as the method of choice for these complex three-dimensional maxillary reconstructions. Mind-Body-Medizin in der integrativen Onkologie

Zusammenfassung

Hintergrund

Weltweit wenden knapp die Hälfte der onkologischen Patienten komplementäre, alternative bzw. integrativ-onkologische Verfahren an. Die Evidenzlage der angewandten Therapien ist sehr inhomogen. Die integrative Onkologie entwickelt eine Kombination aus der konventionell bewährten Medizin (sog. Schulmedizin) und einer wissenschaftlich geprüften Komplementärmedizin/Mind-Body-Medizin (MBM).

Ziel der Arbeit und Fragestellung

Die Studienlage und Einsatzgebiete der Mind-Body-Medizin in der Integrativen Onkologie werden im nachfolgenden Beitrag kurz vorgestellt.

Ergebnisse

Die Wirksamkeit von Entspannungsverfahren zur Reduzierung von Angst und Übelkeit und Verbesserung der Lebensqualität gilt als gut belegt (Level of Evidence 1a). Während und nach Abschluss der primären Therapie zeigt Mindfulness-Based-Stress Reduction (MBSR) positive Effekte. Angst und Stress werden gemindert, die Schlafqualität wird erhöht und die gesundheitsbezogene Lebensqualität gefördert. Ebenfalls gut belegt sind die positiven Wirkungen von Yoga auf die Lebensqualität bei Patientinnen mit Mammakarzinom. Für Qigong und Tai Chi sind positive Effekte auf Fatigue, Schlafqualität, Vitalität und Lebensqualität während und nach der Behandlungsphase dokumentiert. Präoperative Hypnose kann den Verbrauch von Narkosemittel reduzieren sowie die Schmerzempfindung, Übelkeit/Erbrechen, Fatigue und seelische Belastung vermindern.

Schlussfolgerung

Für eine Reihe an Verfahren der MBM besteht eine gute Evidenzlage. Besonders empfohlen werden können 1) Meditation und MBSR zur Steigerung der Lebensqualität, zur Verminderung von Depressivität und für eine verbesserte allgemeine Stressbewältigung, 2) Entspannungstraining zur Verminderung von depressiven Symptomen, 3) Yoga bei Depressionen, Stimmungsschwankungen und Fatigue sowie 4) Hypnose zur Linderung von Fatigue.

Burkitt's lymphoma is a malignant, highly aggressive B-cell type of non-Hodgkin's lymphoma. Prolonged fever followed by bilateral cheek swelling and nasal obstruction was the first symptoms of a 2-year-old boy with Burkitt's lymphoma. When the chemotherapy was started, the facial swelling reduced within few weeks. This case report highlights a rare case of Burkitt's lymphoma in the nasal cavity and bilateral maxillary sinuses and the need of high index of suspicious for early diagnosis and treatment.

Epistaxis is a common complaint seen in the younger population. Its etiology is typically from the anterior nares, although we present a case of epistaxis from an uncommon source. Diagnosis of bleeding nasal mass is varied from benign to malignant lesions. Angiomatous antrochoanal polyps (AAP) are one of the causes but it is rare and the diagnosis is challenging as it mimic other nasal mass especially nasopharyngeal angiofibroma (NA). There are a lot of similarities of clinical presentations, between imaging and histopathological findings between these two which made the diagnosis even more difficult. A correct diagnosis is extremely important as although they have similar clinical presentations, the management is different. Here we report a case of a young lady with significant epistaxis which was initially diagnosed as NA but with a final revised diagnosis of AAP.

Nasofacial NK/T-cell lymphoma is a rare clinical entity characterized by destruction and mutilation of the nasofacial area. It has unknown etiology, often affect face, nose, paranasal sinuses, palate, oral cavity and surrounding structures. As the clinical manifestations are often variable and non-specific, it may obscure the exact diagnosis, so it may delay the treatment. We present a case report of nasofacial NK/T-cell lymphoma in a 56years old male who gave a 3months history of clinical manifestation with destruction of nasofacial area. After confirmed diagnosis, a combined chemotherapy and radiotherapy were offered to the patient who responded well to combination therapy.

T cell receptor repertoire usage in cancer as a surrogate marker for immune responses

Abstract

Characterizing the interaction of cancer cells with the host adaptive immune system is critical for understanding tumor immunology and the modus operandi of immunotherapeutic interventions to treat cancer. As the key cellular effectors of adaptive immunity, T cells are endowed with specialized receptors (the T cell receptor; TCR), to recognize and to eliminate cancer cells. The diversity of the TCR repertoire results from specialized genetic diversification mechanisms that generate an incredible variability allowing recognizing extensive collections of antigens. Based on the attainment and function of the TCR, the TCR repertoire is a mirror of the human immune response, and the dynamic changes of its usage can be assumed as a promising biomarker to monitor immunomodulatory therapies. Recent advances in multiplexed PCR amplification and massive parallel sequencing technologies have facilitated the characterization of TCR repertoires at high resolution even when only biomaterial of limited quantity and quality, such as formalin-fixed paraffin-embedded (FFPE) archived tissues, is available. Here, we review the concept framework and current experimental approaches to characterize the TCR repertoire usage in cancer including inherent technical and biological challenges.

Atypical presentation to rocuronium allergy in a 19-year-old female patient

The danger of anaphylaxis, a rare but life threatening complication of general anesthesia (GA) can be summarized in two: 1. General Anesthesia masks the typical early signs of allergy which can be seen in an awake patient. 2. Increasing body mass index predicts increasing difficulty, failure rate, and time to discovery of failure of epidural anesthesia in laboring patients

Obese parturients both greatly benefit from neuraxial techniques, and may represent a technical challenge to obstetric anesthesiologists. Several studies address the topic of obesity and neuraxial analgesia in general, but few offer well described definitions or rates of "difficulty" and "failure" of labor epidural analgesia. Providing those definitions, we hypothesized that increasing body mass index (BMI) is associated with negative outcomes in both categories and increased time needed for epidural placement.

The use of ultrasound-guided thoracolumbar interfascial plane block for multi-level lumbar spinal surgery

We read with interest the article by Ueshima et al., who indicated that thoracolumbar interfascial plane (TLIP) block was effective for single-level lumbar spinal surgery [1]. TLIP block was reported recently to block dorsal rami of the lumbar spinal nerve by injecting a local anesthetic into the fascial plane between the multifidus and longissimus muscles [2]. However, it is still unclear whether or not TLIP block is available for multi-level lumbar spinal surgery. In this report, we present 2 cases of multi-level lumbar laminectomy performing the TLIP block.

Lower limb amputations performed with anterior quadratus lumborum block and sciatic nerve block

The anterior quadratus lumborum block (QLB) has been used as both a trunk nerve block and a lower limb nerve block [1,2]. Compared with lumber plexus nerve block, anterior QLB can be safety performed on patients who take anticoagulant because of a shallow nerve block. We report two successful cases of lower limb amputation with an anterior QLB on a patient who had previously lumber surgery. Case 1 was a 75-year man (173cm, 78kg) who was scheduled for a right above-knee amputation because of arteriosclerosis obliterans.

Not all mucous plugs are created equal!

Mucous plug related airway obstruction is a very common clinical problem in patients on endotracheal tube and tracheostomy tube (TT). The obstruction can be catastrophic in smaller children where the lumen of the artificial airway is already small to begin with and any minimal mucous plug can produce a complete obstruction. We present the case of a mucous plug related complete airway obstruction which encountered no resistance with passage of a suction catheter, but rather was found to be acting like a ball valve.

Clinical experience of anterior quadratus lumborum block after lumber surgery

Anterior quadratus lumborum block (QLB) is reported as one of trunk nerve block [1]. In addition, because anterior QLB injects a local anesthetic between the psoas (PM) and the quadratus lumborum (QB), the anterior QLB may be able to block lumbar plexus nerves. Therefore it may be effective for a postoperative pain of lower surgery [2]. In this time, we report one successful case of open reduction internal fixation (ORIF) of left femur fracture by performing only the anterior QLB for the patient with a lumber surgery.

This study aimed to review research on the effects of nonsteroidal anti-inflammatory drugs (NSAIDs) on β-endorphin. NSAIDs are commonly used as anti-inflammatory and analgesic drugs. They are well known for inducing peripheral analgesia by inhibiting cyclooxygenase (COX). However, an increasing number of studies have shown that NSAIDs have an analgesic effect not only in the periphery but also at the center. It means that a central analgesic mechanism of the action of NSAIDs exists besides the peripheral mechanism, and the central mechanism likely involves β-endorphin.

Management of isolated infraorbital neuralgia by ultrasound-guided infraorbital nerve block with combination of steroid and local anesthetic

Optimal site for the subpectoral interfascial plane block

The Subpectoral interfascial plane (SIP) block, which was first described in 2016 by Raza et al. [ 1], involves the injection of a local anesthetic into the space between the pectoral major muscle (PMM) and the external intercostal muscle (EIM). It anesthetizes the anterior rami of the T2 to T6; therefore, SIP block may be effective for the postoperative analgesia of the median sternotomy such as that following a cardiac surgery [2]. With great interest, we have read the article by Kutanis et al. entitled "Dexmedetomidine acts as an oxidative damage prophylactic in rats exposed to ionizing radiation" that is published in the Journal of Clinical Anesthesia (2016) 34, 577–585 [1]. Kutanis et al. investigated the effects of dexmedetomidine on oxidative injury caused by ionizing radiation. The authors concluded that Dexmedetomidine preserves the antioxidant enzyme levels and reduces toxic oxidant metabolites caused by ionizing radiation.

Use of test dose allows early detection of subdural local anesthetic injection with lumbar plexus block

A 56year-old woman underwent a lumbar plexus block for a revision of a left total hip arthroplasty. During the block procedure, the needle was advanced over the transverse process and isolated quadriceps twitches were elicited. After administering a test dose of 3ml of 1.5% lidocaine, the patient developed loss of sensation in the L3–4 dermatomal distribution that progressed caudally to involve both legs followed by inability to move the left leg. The patient shortly thereafter became hypotensive and sensory block spread cephalad and peaked at C7 bilaterally suggesting possible subdural spread of local anesthetic.

Lumbar vertebra surgery performed with a bilateral retrolaminar block

The number of lumbar vertebrae surgeries has increased in recent years. Despite this increase, the methods for perioperative pain management have remained limited. A retrolaminar block has been reported to be an effective analgesic in breast cancer surgery, transapical transcatheter aortic valve implantation, and ribs fracture treatment [1,2,3]. However, the use of a retrolaminar block for lumbar vertebra surgery has not been reported. Use of Sugammadex in an octagenerian with Myaesthenia Gravis undergoing emergency laporotomy

Myaesthenia Gravis is an autoimmune disorder that results in increased sensitivity to neuromuscular blockers. Anaesthesia and surgery in patients with Myaesthenia Gravis is associated with an increased incidence of complications, including post operative ventilation. We report the successful use of intra-operative train of four ratio monitoring and Sugammadex to reverse rocuronium induced neuromuscular blockade in an elderly octagenarian with Myaesthenia Gravis undergoing an emergency laporotomy.

Multiple Oral Mucosal Hamartomas in a 34-Year Old Female

Abstract

A case of Cowden syndrome (CS) is described in a 34-year-old African American female who reported a history of breast and thyroid malignancies. Clinical examination demonstrated multiple soft, white-pink papules across multiple mucosal surfaces of the oral cavity. Microscopy of the lesions revealed hyperkeratotic surface squamous epithelium with papillomatosis and acanthosis along with elongated rete processes. A genomic polymerase chain reaction direct sequencing using the patient's blood was positive for mutations of the PTEN gene typical of CS.

Der Buschke-Löwenstein-Tumor (BLT) ist eine eigenständige Entität und bezeichnet Condylomata acuminata mit einem übermäßigen, lokal destruierenden, aber nicht invasiven Wachstum.

Fragestellung

Existieren neue Erkenntnisse zur pathognomonischen Einordnung und neue therapeutische Empfehlungen?

Material und Methode

Ein Literaturüberblick über Fallberichte der letzten 10 Jahre wird gegeben.

Ergebnis

BLT sind durch ihre virale Genese vom verrukösen Karzinom abzugrenzen. In den letzten 10 Jahren wurden in der Medline-Datenbank knapp über 100 neue Fälle beschrieben. Eindeutige Zeichen der Malignität im Sinne von invasivem Wachstum fanden sich in diesem Patientengut bei 12 % der Erkrankten. In 14 % bestand eine HIV-Infektion. Da das invasive Wachstum meistens nur fokal auftritt und es daher nur nach kompletter Exzision des Tumors diagnostizierbar ist, muss der BLT wie ein Karzinom behandelt und als niedrigmalignes, aus Condylomata acuminata entstandenes In-situ-Karzinom definiert werden. Zur Diagnosestellung des BLT gehören neben der histologischen Sicherung der Virusnachweis sowie der Ausschluss von Invasion und Metastasierung durch Schnittbildgebung. Die organerhaltende, operative Resektion ist die Therapie der Wahl.

Schlussfolgerungen

BLT stellen niedrigmaligne, aus Condylomata acuminata entstandene In-situ-Karzinome mit einer hohen Neigung zur fokalen Invasion, aber geringem Metastasierungspotenzial dar. Die organerhaltende Operation ist anzustreben.

Biological kinship, postmarital residence, and the emergence of cemetery formalization at prehistoric Marathon

Abstract

This paper examines the role of kinship and postmarital residence in the emergence of organized cemeteries during the transition from the Final Neolithic to the Early Bronze Age at Marathon, Attica. Focusing on the cemetery of Tsepi, we performed intra-cemetery biodistance analysis to test whether biological relatedness structured spatial organization of tombs, and whether postmarital residence was matrilocal or patrilocal. Dental metric, dental morphological, and cranial non-metric data were collected from 293 individuals and subjected to multivariate analysis (PCA, Euclidean distance ordination via MDS), binomial probability calculations, Ripley's K analysis, and determinant ratio analysis. Results indicated phenotypic similarity among some tomb co-interments, however, outliers suggested that within-tomb burial depended on a variety of factors that could include affinal, fictive, or practical kinship. There was strong evidence for phenotypic patterning by tomb row, indicating that cemetery structure at Tsepi was organized according to biological lineages. This was especially evident for females. Male phenotypic variation was higher than that of females, though not significantly so. The results of intra-cemetery biodistance analysis at Tsepi reveal a complex mortuary program that emphasized biological kinship within an exogamous and likely matrilocal system of mate exchange. When considered in light of ethnographic evidence, the practice of male exogamy may correlate with the coastal location of the community and the maritime activities that structured the economy during the emergence of social complexity in the region. This paper illustrates the potential of biodistance analyses for elucidating aspects of social life in the Aegean.

Childhood Cranial Trauma from a late Roman and Merovingian context from Michelet, Lisieux, France

The Michelet necropolis in Lisieux, France, dating to the late Roman and Merovingian period, comprises of a large number of well-preserved subadult remains offering a unique opportunity to better understand childhood trauma in the past. The focus of this study was to determine the amount, type, and mechanisms of trauma evidenced in subadults from the 4th–8th century AD, and explore potential circumstances surrounding the trauma. The remains of 109 subadults from the Michelet necropolis were examined for the presence of cranial and post-cranial trauma. Three individuals exhibited perimortem trauma, one individual had an antemortem cranial injury, and no cases of post-cranial trauma were identified. Cranial trauma affected 4.1% of children with observable cranial remains (N = 4/97). The children affected were young (2-7 years old), making it unlikely that they would have participated in militaristic activities. Based on the location, morphology and mechanism of injury identified, it is likely that the perimortem injuries sustained by three children were not accidental. The presence of a number of cranial injuries from this site may be related to increased stress in the community related to the decline of the Roman Empire in Gaul, possible raiding barbarian groups during the 4th–5th centuries, or stresses related to the Gallic aristocracy solidifying political powers in northern Gaul during the 5th–7th centuries AD. The consideration and inclusion of childhood trauma in bioarchaeological analyses allows for a more detailed and in-depth understanding of violence and childhood in the past.

Donor-derived mycosis fungoides following reduced intensity haematopoietic stem cell transplantation from a matched unrelated donor

A 46-year-old woman with a history of dasatinib-resistant chronic myeloid leukaemia, clonal evolution and monosomy 7 underwent reduced intensity conditioned in vivo T-cell-depleted allogeneic haematopoietic stem cell transplantation (HSCT) from a matched unrelated donor. Following the transplantation, she developed recurrent cutaneous graft versus host disease (GvHD), which required treatment with systemic immunosuppression and electrocorporeal photophoresis. Concurrently, she developed a lichenoid rash with granulomatous features suggestive of cutaneous sarcoidosis. Additional treatment with hydroxychloroquine was initially successful, but 2 months later, she developed erythroderma with palpable lymphadenopathy. Repeated histological analysis established a diagnosis of folliculotropic mycosis fungoides stage IVA2, and the malignant clone was confirmed to be of donor origin. A positive response to brentuximab has been shown. This is the first reported case of primary mycosis fungoides after matched unrelated donor HSCT, and in a patient still undergoing treatment for GvHD.

PurposeThe aim of this cadaveric study was to assess the accuracy of the computer-assisted template-guided autotransplantation of teeth with custom 3D designed/printed surgical tooling.MethodsTen partially edentulous human mandibular cadavers were scanned with a cone-beam computed tomography (CBCT) system and intra-oral scan system. The 3D data of these cadavers were imported in specialized software and used to analyze the region of the recipient sites and the donor teeth were selected. Subsequently, congruent to the donor teeth, custom surgical tooling and surgical guided templates were designed and 3D printed. The guided osteotomies were performed and the donor teeth transplanted. To evaluate the planned donor teeth positions in comparison to the transplanted donor teeth positions the mandibles were scanned again with the CBCT system and software matching was applied to measure the accuracy of the procedure.ResultsThe mean angular deflection of the transplanted donor teeth with the planned donor teeth positions was 5.6° (5.4 SD). When comparing the 3D positions of the shoulders there is a mean deviation of 3.15 mm (1.16 SD) and a mean apical deviation of 2.61 mm (0.78 SD).ConclusionThe described method of computer-assisted template-guided autotransplantation of teeth with custom 3D designed/printed surgical tooling could potentially provide a relatively accurate alternative for the currently available treatment approaches. Further research should focus on improving the accuracy of this technique and evaluating clinical success and advantages of this method.

ObjectiveCentral giant cell granulomas (CGCGs) are clinically classified as non-aggressive (nA-CGCG) and aggressive (A-CGCG). However, histopathologically all lesions feature spindle mononuclear cells (MCs) and multinuclear giant cells (GCs) in a hemorrhage-rich stroma. We aimed to investigate presence of cells with a monocyte/macrophage-related phenotype and together with clinical variables to examine their predictive potential for the biological behavior of CGCG.Patients and MethodsInvestigation was implemented a retrospective cohort study. Sections were immunohistochemically stained for colony stimulating factor–1 receptor (CSF-1R)/CD115, CD163, CD68 and nuclear factor kappa B (NF-kB). The clinical variables included age, gender and location of lesions. Association between immunostains and clinical variables and CGCG aggressiveness were analyzed by Wilcoxon–Mann–Whitney exact test and t-test. Significant variables were further analyzed by a logistic regression model followed by receiver operating characteristic (ROC) curve for diagnostic sensitivity. Significance was set at P < .05.ResultsPatients with A-CGCG (n = 36) were younger than those with nA-CGCG (n = 31) (P = .002). Logistic regression showed that CD163-GC (β = - .870, P = .031) and CD115-MC (β = - .783, P = .027) had a significant protection effect (OR = .419, 95% CI.190 thru .925, and OR = .457, 95% CI .229 thru .913, respectively). ROC curve analysis showed that CD163-GC and CSF-1R/CD115-MC combined were the best predictor in distinguishing nA-CGCG from A-CGCG (area under ROC, 0.814; P < .001). At the optimal cutoff value (0.408), sensitivity was 87% and specificity 65%.ConclusionIncreasing age and high expression of CD163-GC and CSF-1R/CD115-MC can serve as significant predictors of nA-CGCG. A functional link between CD163-GC and the characteristic areas of extravasation of erythrocyte is discussed.

PurposeDexamethasone (dex) seems to suppress postoperative swelling. However, the standard administration dose of dex for bilateral sagittal split osteotomies (BSSOs) has not been reported. This study focused to understand the most effective dose of dex for BSSOs.Materials and MethodsThis research was planned as a prospective randomized controlled double-blind study. Patients undergoing BSSOs were randomly assigned and received intravenous preoperative dex under 3 different dose conditions: 16 mg, 8 mg, and 0 mg (control). The endpoints of this study were postoperative changes in a) masseter muscle thickness and buccal soft tissue, b) maximum incisal opening, c) sensation of the chin and lower lip region, d) blood examinations (WBCs, neutrophils, CRP, and lymphocytes), and d) types of complications. Data were recorded at 2 to 4 time intervals: i) before surgery); ii) postoperative day 1; iii) postoperative day 2; iv) postoperative day 3. Average age, gender, average body mass index, average surgery time, and average blood loss were also examined. Data were analyzed using one-way analysis of variance (Bonferroni's Multiple Comparison Test) after Bartlett's test.ResultsTwenty-four patients including 5 men and 19 women were enrolled in this study. The rate of increase (ROI) in the thickness of masseter muscle 24 hours after BSSOs was 38.4% in the 16 mg group (n = 8), 57.7% in the 8 mg group (n = 8), and 56.1% in the 0 mg group (n = 8). The ROI in the thickness of masseter muscle in the 16 mg group was significantly lower than that in the 0 mg group (P < .05). The number of lymphocytes after surgery shown that the 16 mg and 8 mg groups maintained preoperative levels in the number of lymphocytes, whereas there was a reduced number of lymphocytes in the control group. No statistically significant results were obtained for the study endpoints b) and c).ConclusionThis investigation demonstrated that the most effective dose of dex for BSSOs is 16 mg.

Successful pregnancy following treatment of recurrent chronic histiocytic intervillositis

Chronic histiocytic intervillositis (CHI) is a rare placental lesion associated with adverse obstetric outcomes and high recurrence rate. We report a case of six consecutive pregnancies in one woman, where CHI was detected following an intrauterine death in the fifth pregnancy, after being missed in four earlier losses. The successful sixth pregnancy was treated with a combination of immunosuppressive and antithrombotic agents. While low-molecular-weight heparin (LMWH) and aspirin had been shown to improve pregnancy outcome in recurrent pregnancy loss, there was limited evidence of improved outcome in CHI. It has been suggested that CHI may result from a maternal immunological process and there have been a few reports of the use of corticosteroids because of this possibility, though without convincing evidence of efficacy. Foamy gland variant of adenocarcinoma of prostate: a rare pathological variant

Description

A 58-year-old man presented with history of irritative lower urinary tract symptoms (daytime frequency and nocturia) for 3 months. Digital rectal examination revealed grade 2 prostatomegaly and consistency was hard. Serum prostate-specific antigen (PSA) was 2.7 ng/mL. Serum creatinine was 1.2 mg/dL. Transrectal ultrasonography (TRUS) revealed a prostate size of 54x47x56 mm with hypoechoic nodular appearance at superior part of the left lobe. Ultrasound scan of the abdomen was normal. Urinary microscopy revealed 1–2 pus cells and 3–4 erythrocytes. TRUS-guided 12-core biopsy was performed. Microscopic examination of all the biopsy cores revealed fused glandular structures with focal cribriform pattern (Gleason pattern 4+4=8), involving >80% of the prostatic cores (figures 1 and 2). Perineural invasion was evident. Individual cells were polygonal with voluminous foamy cytoplasm, low nucleo-cytoplasmic ratio, pyknotic nuclei and inconspicuous nucleoli. Pancreatic lymphoma complicating early stage chronic hepatitis C

Hepatitis C virus (HCV) infection has also been associated with many extrahepatic manifestations including the development of B-cell non-Hodgkin's lymphoma (NHL). Primary pancreatic lymphoma is very rare and comprises 2.2% of NHL and 4.9% of all pancreatic malignancies. Our patient was a woman with a history of infection with HCV found to have a mass in the head of the pancreas. Biopsy of the mass revealed a high-grade B-cell lymphoma consistent with Burkitt's lymphoma. Our case reflects a need to initiate antiviral therapy for all patients infected with HCV even in early stages of fibrosis to prevent cirrhosis and other extrahepatic manifestations of infection with HCV.

Radial artery pseudoaneurysm: rare complication of a frequent procedure

Description

The pseudoaneurysms of the radial artery are uncommon, generally secondary to trauma or invasive procedures such as arterial catheterisation (0.09%).12 Predisposing factors are advanced age, longer duration of catheterisation, coagulation disorders or use of anticoagulants/antiplatelet agents. A woman of 85 years, with history of ischaemic heart failure, chronic atrial fibrillation on anticoagulation, with labile International Normalisation Ratio, hypertension, peripheral artery disease and dyslipidaemia, was admitted in the context of acute pulmonary oedema secondary to hypertension, requiring hospitalisation in intermediate care unit and continuous arterial catheterisation. Successful treatment of Takayasu arteritis with rituximab as a first-line immunosuppressant

Takayasu arteritis is a rare large vessel vasculitis which has traditionally been treated with high-dose steroids. There have been a small number of publications where biological agents have been used to manage refractory cases. To the authors knowledge, there are no publications using biological agents in combination with steroids as a first-line treatment in Takayasu arteritis. In this publication, we document the case of Takayasu arteritis, in a 39-year-old woman, where rituximab was used in combination with steroids as a first-line agent in the setting of poorly controlled bipolar affective disorder.

Recurrent angioedema associated with pharmacological inhibition of dipeptidyl peptidase IV

Angioedema (AE) of the upper airways is a severe and potentially life-threatening condition. The incidence has been increasing in the past two decades, primarily due to increased use of medications inhibiting the degradation of vasoactive peptides. Acquired angioedema related to angiotensin-converting enzyme inhibitors (ACEI-AAE) is well known, but other pharmaceutical agents also affect the degradation of bradykinin and substance P. We present a middle-aged man with recurrent episodes of severe AE of the oral cavity, hypopharynx and larynx due to pharmacological inhibition of dipeptidyl peptidase IV.

Benign cystic mediastinal teratoma presenting as a massive pleural effusion in a 17-year-old boy

Mediastinal teratomas are a rare, albeit an important differential diagnosis of anterior/middle mediastinal masses in young adults and various atypical presentations have been reported. The authors report a case of a 17-year-old boy who presented with a 2-month history of worsening shortness of breath and pleuritic chest pain. A massive left-sided pleural fluid collection was seen on a chest radiograph (CXR). The pleural fluid was drained and a CT Thorax confirmed the presence of a cystic mass. Following re-accumulation of the fluid, thoracotomy was performed and a benign mediastinal teratoma excised. The patient remained well with no evidence of recurrence on follow-up CXRs a year post operatively.

Unexpected complication after radical (inguinal) orchidectomy: trans-sphincteric anoscrotal fistula

Anoscrotal fistula is an extremely rare condition involving an epithelialised tract between the anal canal and scrotal wall. It is more commonly seen as a congenital phenomenon in the paediatric population, and has not previously been described in the literature in adults. We present the case of a man aged 52 years who developed a complex anoscrotal fistula after a radical inguinal orchidectomy for an intratesticular seminoma. A postsurgical wound-site infection developed into a chronically discharging wound refractory to antibiotic treatment prompting further investigation. By way of MRI, a 10 cm long trans-sphincteric anoscrotal fistula was found. The patient was successfully treated by fistuloscopic curettage, internal closure using an Ovesco clip, and negative pressure dressing. We present for the first time a rare anoscrotal fistula in an adult.

Gambling disorder: a side effect of an off-label prescription of baclofen--literature review

The use of high-dose baclofen emerged in 2008 in the treatment of alcohol-use disorders. Its prescription is still off-label in France, but recent trials have suggested the interest of using high doses for alcohol dependence, so we have to deal with an increase in its use. However, we still have few data about the adverse effects of a high-dose baclofen prescription, especially in complex addictive disorders. We present a case of a 32-year-old man who sought treatment for gambling disorders (GDs). He had complex addictive disorders, including alcohol-use disorders and GDs. He developed a severe GD, after treatment with a high dose of baclofen. The maximum dose was 160 mg/day, prescribed for his alcohol-use disorders. According to the Naranjo algorithm, the score was +7, it enabled to conclude that problem of gambling was probably imputable to baclofen. We discuss this case with reference to literature.

Pacemaker pocket infection due to Mycobacterium goodii, a rapidly growing mycobacteria

A woman aged 74 years with an implanted dual-chamber pacemaker presented with pacemaker site infection after failing empiric antimicrobial therapy. The pathogen was later identified as Mycobacterium goodii, a rapidly growing mycobacteria species. The pacemaker was subsequently removed and the patient was treated with oral ciprofloxacin and doxycycline with clinical improvement. In this article, we describe a rare case of pacemaker site infection by M. goodii.

Superficial temporal artery haemorrhage caused by neurophysiological monitoring: a unique MRI finding

A woman aged 66 years with a history of diplopia, headaches and left Cranial Nerve 3 palsy was found to have a 2 cm left paraclinoid internal carotid artery aneurysm. She was started on aspirin and clopidogrel and underwent flow diversion treatment with Pipeline (Covidien Neurovascular, Irvine, California, USA) embolisation of the aneurysm. Intraoperative neurophysiological monitoring via EEG and somatosensory evoked potentials was performed. After the procedure, she was found to have a large right scalp haematoma extending to the right orbit anteriorly and the back of the head posteriorly. Despite manual compression, the periorbital swelling continued to increase in the intensive care unit such that she was not able to open the right eye. Healing of pathological fracture in a case of multiple myeloma

Description

A man aged 41 years presented to us with a lytic lesion in the proximal humerus and developed a pathological fracture which was managed with nailing (figure 1). He was eventually diagnosed with multiple myeloma.1 This patient underwent autologous bone marrow transplant. He was also given chemotherapy using a regimen of bortezomib, lenalidomide and dexamethasone. The humeral fracture showed sound union at the follow-up of 1.5 years with the lesion completely healed (figure 2).

Combined central retinal artery and vein occlusion in lupus

Description

A girl aged 16 years presented with sudden loss of vision in the left eye (LE) for 1 day. Visual acuity was 6/6 in the right eye (RE) and perception of light in LE. LE had relative afferent pupillary defect. RE fundus had cotton wool spots (figure 1A). LE fundus showed combined central retinal artery occlusion (CRAO) and central retinal vein occlusion (CRVO) (figure 1B). Her history of alopecia, fever and arthritis for 6 months, bilateral cervical adenopathy, hard palate ulcers, leucopenia (2300/mm3), lymphopenia (621/mm3), positive antinuclear antibody and undetectable serum C4 led us to diagnose systemic lupus erythematosus (SLE). Lupus anticoagulant, anticardiolipin (IgM, IgG) and anti-β2 glycoprotein1 (IgM, IgG) antibodies were negative. She was treated with high-dose corticosteroids and anticoagulant therapy. Delayed diagnosis of fractured anterior arch of the atlas in a young child

A 2-year-old girl fell off a bunk bed onto a parquet floor. She immediately reported neck pain and presented with muscle spasm and limited motion of the cervical spine (C-spine). Plain X-rays of the C-spine showed no osseous lesion. Owing to persisting pain and limited motion in the neck, MRI of the C-spine was obtained which revealed intact ligaments and cervical spinal cord, as well as soft tissue swelling in front of the anterior arch of the atlas. Subsequent CT of the C-spine confirmed a complete, undisplaced fracture of the anterior arch of the atlas (Gehweiler type I fracture). A Minerva cast was applied for 2 months, followed by a soft cervical collar. Persistent neck pain and limited range of motion of the neck after a fall may be indicative of atlas fracture that should be ruled out by CT.

Pilot study of mobile phone technology in allergic rhinitis in European countries. The MASK-rhinitis study

Abstract

Background

The use of Apps running on smartphones and tablets profoundly affects medicine. The MASK-rhinitis (MACVIA-ARIA Sentinel NetworK for allergic rhinitis) App (Allergy Diary) assesses allergic rhinitis symptoms, disease control and impact on patients' lives. It is freely available in 20 countries (iOS and Android platforms).

Aims

To assess in a pilot study whether (i) Allergy Diary users were able to properly provide baseline characteristics (ii) simple phenotypic characteristics based upon data captured by the Allergy Diary could be identified and (iii) information gathered by this study could suggest novel research questions.

Methods

The Allergy Diary users were classified into 6 groups according to the baseline data that they entered into the App: (1) asymptomatic; (2) nasal symptoms excluding rhinorrhea; (3) rhinorrhea; (4) rhinorrhea plus 1-2 nasal/ocular symptoms; (5) rhinorrhea plus ≥3 nasal /ocular symptoms and (6) rhinorrhea plus all nasal/ocular symptoms.

Results

By June 1, 2016, 3,260 users had registered with the Allergy Diary and 2,710 had completed the baseline questionnaire. Troublesome symptoms were found mainly in the users with the most symptoms. Around 50% of users with troublesome rhinitis and/or ocular symptoms suffered work impairment. Sleep was impaired by troublesome symptoms and nasal obstruction.

Conclusions

This is the first App (iOS and Android) to have tested for allergic rhinitis and conjunctivitis. A simple questionnaire administered by cell phones enables the identification of phenotypic differences between a priori defined rhinitis groups. The results suggest novel concepts and research questions in allergic rhinitis that may not be identified using classical methods.

Two Cases of Ectopic Hamartomatous Thymoma Masquerading as Sarcoma

Ectopic hamartomatous thymoma (EHT) is an extremely rare benign tumor. EHTs are difficult to differentiate from sarcomas, especially synovial sarcomas. We encountered two cases of EHT that were referred from other hospitals because sarcoma was suspected. In these cases, fusion gene detection via polymerase chain reaction or fluorescence in situ hybridization was useful for differentiating EHT from synovial sarcoma. EHT requires accurate diagnosis before surgery to avoid excessive treatment. Both tumor location and the presence of fat inside the tumor are important imaging findings for EHT, and confirmation of spindle cells, epithelial cells, and mature adipose cells in the tumor is an important pathological finding. It is important to exclude synovial sarcoma from the differential diagnosis via fusion gene analysis.

Interferon α (IFN-α) exerts potent antiviral, immunomodulatory, and antiproliferative activity and have proven clinical utility in chronic hepatitis B and C virus infections. However, repeated IFN-α administration induces neutralizing antibodies (NAb) against the therapeutic in a significant number of patients. Associations between IFN-α immunogenicity and loss of efficacy have been described.So as to improve the invivo biological efficacy of IFN-α, a long lasting hyperglycosylated protein (4N-IFN) derived from IFN-α2b wild type (WT-IFN) was developed. However, in silico analysis performed using established in silico methods revealed that 4N-IFN had more T cell epitopes than WT-IFN. In order to develop a safer and more efficient IFN therapy, we applied the DeFT (De-immunization of Functional Therapeutics) approach to producing functional, de-immunized versions of 4N-IFN.Using the OptiMatrix in silico tool in ISPRI, the 4N-IFN sequence was modified to reduce HLA binding potential of specific T cell epitopes. Following verification of predictions by HLA binding assays, eight modifications were selected and integrated in three variants: 4N-IFN(VAR1), (VAR2) and (VAR3). Two of the three variants (VAR1 and VAR3) retained anti-viral function and demonstrated reduced T-cell immunogenicity in terms of T-cell proliferation and Th1 and Th2 cytokine levels, when compared to controls (commercial NG-IFN (non-glycosylated), PEG-IFN, WT-IFN and 4N-IFN).It was previously demonstrated that N-glycosylation improved IFN-α pharmacokinetic properties. Here, we further reduce immunogenicity as measured invitro using T cell assays and cytokine profiling by modifying the T cell epitope content of a protein (de-immunizing). Taking into consideration the present results and previously reported immunogenicity data for commercial IFN-α2b variants, 4N-IFN(VAR1) and 4N-IFN-4N(VAR3) appear to be promising candidates for improved IFN-α therapy of HCV and HBV.

Inflammatory Duodenal Polyposis Associated with Primary Immunodeficiency Disease: A Novel Case Report

Agammaglobulinemia is a rare form of B-cell primary immunodeficiency disease characterized by reduced levels of IgG, IgA, or IgM and recurrent bacterial infections. Agammaglobulinemia is most commonly associated with diffuse nodular lymphoid hyperplasia. Duodenal polyps are a rare entity; however, due to wide use of esophagogastroduodenoscopy, incidental diagnosis of duodenal polyps appears to be increasing. Although inflammatory duodenal polyposis has been reported in the literature, its association with common variable immunodeficiency has not been reported till date to the best of our knowledge. We report a case of a 59-year-old male with chronic symptoms of agammaglobulinemia associated with inflammatory duodenal polyposis.

BackgroundIn asthma remodeling airway smooth muscle cells (ASMCs) contribute to airway wall thickness through increased proliferation, migration, and extracellular matrix deposition. Previously, we described that protein arginine methyltransferase 1 (PRMT1) participates in airway remodeling in pulmonary inflammation in E3 rats.ObjectiveWe sought to define the asthma-specific regulatory mechanism of PRMT1 in human ASMCs.MethodsASMCs from healthy subjects and asthmatic patients were activated with platelet-derived growth factor (PDGF)–BB. PRMT1 was localized by means of immunohistochemistry in human lung tissue sections and by means of immunofluorescence in isolated ASMCs. PRMT1 activity was suppressed by the pan-PRMT inhibitor AMI-1, signal transducer and activator of transcription 1 (STAT1) was suppressed by small interfering RNA, and extracellular signal-regulated kinase (ERK) 1/2 mitogen-activated protein kinase (MAPK) was suppressed by PD98059. MicroRNAs (miRs) were assessed by using real-time quantitative PCR and regulated by miR mimics or inhibitors.ResultsPRMT1 expression was significantly increased in lung tissue sections and in isolated ASMCs of patients with severe asthma. PDGF-BB significantly increased PRMT1 expression through ERK1/2 MAPK and STAT1 signaling in control ASMCs, whereas in ASMCs from asthmatic patients, these proteins were constitutively expressed. ASMCs from asthmatic patients had reduced miR-19a expression, causing upregulation of ERK1/2 MAPK, STAT1, and PRMT1. Inhibition of PRMT1 abrogated collagen type I and fibronectin deposition, cell proliferation, and migration of ASMCs from asthmatic patients.ConclusionsPRMT1 is a central regulator of tissue remodeling in ASMCs from asthmatic patients through the pathway: PDGF-BB–miR-19a–ERK1/2 MAPK and STAT1. Low miR-19a expression in ASMCs from asthmatic patients is the key event that results in constitutive increased PRMT1 expression and remodeling. Therefore PRMT1 is an attractive target to limit airway wall remodeling in asthmatic patients.

Use of TNF-inhibitors and ustekinumab for psoriasis during pregnancy: A patient series

Abstract

From 2002 to 2016 a total of seven women with severe refractory psoriasis were exposed to the TNF-inhibitors infliximab and adalimumab or to the IL12/23 inhibitor ustekinumab during one or more pregnancies. Maternal, fetal or teratogenic toxicity were not detected during pregnancy and puerperium. All pregnancies were uneventful and resulted in delivery of 10 healthy children in total, one of the women is due February 2017. Postpartum, five of the women were lactating, but none of the women or newborns developed adverse reactions. Data on safety of treatment during breastfeeding are sparse, but so far appears to be safe due to the lack of absorption across the gastrointestinal lining. Currently biological therapy with either TNF-inhibitors or ustekinumab is not recommended during pregnancy, however in selected women with severe psoriasis these treatment modalities may be considered.

Outcomes Following Conversion of a Percutaneous to a Transcutaneous Bone Conduction Device in Eight Children

Abstract

The percutaneous bone conduction device (P-OBCD) is a well-established method of hearing rehabilitation. However, it is associated with soft tissue complications involving the peri-abutment skin.

The magnetic osseointegrated bone conduction (T-OBCD) device overcomes this by leaving an intact skin flap over the implant.

The current study assesses the outcomes of conversion to a T-OBCD in a paediatric population in two tertiary referral paediatric centres.

Eight children underwent conversion. The mean age at conversion was 10 years (range 5-16 years). All had skin reactions between Holgers 3 and 4. Mean improvement in thresholds was 25dBHL.

Conversion from a P-OBCD to a T-OBCD is a safe and beneficial method of hearing rehabilitation in children with persistent soft tissue complications.

Synergistic effects of host B7-H4 deficiency and gemcitabine treatment on tumor regression and anti-tumor T cell immunity in a mouse model

Abstract

B7-H4 (B7x/B7S1), a B7 family inhibitor of T cell activity, is expressed in multiple human cancers and correlates with decreased infiltrating lymphocytes and poor prognosis. In murine models, tumor-expressed B7-H4 enhances tumor growth and reduces T cell immunity, and blockade of tumor-B7-H4 rescues T cell activity and lowers tumor burden. This implicates B7-H4 as a target for cancer immunotherapy, yet limits the efficacy of B7-H4 blockade exclusively to patients with B7-H4+ tumors. Given the expression of B7-H4 on host immune cells, we have previously shown that BALB/c mice lacking host B7-H4 have enhanced anti-tumor profiles, yet similar 4T1 tumor growth relative to control. Given that T cell-mediated immunotherapies work best for tumors presenting tumor-associated neoantigens, we further investigated the function of host B7-H4 in the growth of a more immunogenic derivative, 4T1-12B, which is known to elicit strong anti-tumor CD8 T cell responses due to expression of a surrogate tumor-specific antigen, firefly luciferase. Notably, B7-H4 knockout hosts not only mounted greater tumor-associated anti-tumor T cell responses, but also displayed reduced tumors. Additionally, B7-H4-deficiency synergized with gemcitabine to further inhibit tumor growth, often leading to tumor eradication and the generation of protective T cell immunity. These findings imply that inhibition of host B7-H4 can enhance anti-tumor T cell immunity in immunogenic cancers, and can be combined with other anti-cancer therapies to further reduce tumor burden regardless of tumor-B7-H4 positivity.

Intrahepatische Schwangerschaftscholestase

Zusammenfassung

Die intrahepatische Schwangerschaftscholestase ist eine schwangerschaftsspezifische Lebererkrankung, die durchschnittlich in etwa 0,5–2,0 % aller Schwangerschaften auftritt. Sie zeigt meist einen benignen Verlauf für die Mutter und ist durch maternalen Pruritus v. a. im dritten Trimester, erhöhte Transaminasen und Nüchtern-Gallensalzkonzentrationen im Serum sowie ein gesteigertes fetales Risikoprofil charakterisiert. Die Ätiologie der intrahepatischen Schwangerschaftscholestase ist bisher nur teilweise geklärt, scheint aber multifaktoriell bedingt zu sein. Cholestase verursachende Effekte bestimmter weiblicher Schwangerschaftshormone und deren Metabolite spielen dabei eine entscheidende Rolle bei genetisch prädisponierten Frauen. Ebenfalls sind die Mechanismen unbekannt, die für die fetalen Komplikationen wie spontane Frühgeburten, Mekoniumfärbung des Fruchtwassers, Neugeborenenasphyxie bis hin zu fetalen Todesfällen verantwortlich sind. Für die Pathogenese des Pruritus in der Schwangerschaftscholestase scheinen bestimmte sulfatierte Progesteronmetabolite verantwortlich zu sein. Im Gegensatz zu Pruritus bei schwangerschaftsassoziierten Dermatosen liegen bei Patienten mit intrahepatischer Schwangerschaftscholestase keine primären Hautveränderungen vor. Allerdings kann intensives Kratzen zu sekundären Hautveränderungen wie Erosionen, Exkoriationen, Krusten und selten auch zu Prurigo nodularis führen. Die Behandlung basiert entsprechend den aktuellen Leitlinien in der oralen Gabe von Ursodeoxycholsäure und der teils praktizierten elektiven Geburtseinleitung in der 37. bis 38. Schwangerschaftswoche, um möglichen fetalen Risiken, insbesondere der Totgeburt, zuvorzukommen. Dieser Übersichtsbeitrag fasst Epidemiologie, klinische Symptome, Diagnose, Behandlung und insbesondere die Pathogenese des Pruritus zusammen.

Spezifische Schwangerschaftsdermatosen

Zusammenfassung

Die spezifischen Schwangerschaftsdermatosen repräsentieren eine heterogene Gruppe entzündlicher Dermatosen, die ausschließlich mit der Schwangerschaft und/oder der Postpartalperiode vergesellschaftet sind. Eine klinikrelevante Klassifikation hat sich über die letzten 10 Jahre etabliert und unterscheidet die folgenden 4 Erkrankungen: Pemphigoid gestationis, polymorphe Schwangerschaftsdermatose, intrahepatische Schwangerschaftscholestase und atopische Schwangerschaftsdermatose. Ihr gemeinsames Leitsymptom ist heftiger Pruritus, der von charakteristischen Effloreszenzen begleitet wird. Während einige dieser Dermatosen aufgrund des Pruritus lediglich unangenehm für die Mutter sind, bergen andere ein potenzielles Risiko für das Kind. Frühzeitige Diagnose und spezifische Therapie sind daher unerlässlich. Diese Übersicht widmet sich dem Pemphigoid gestationis, der polymorphen sowie der atopischen Schwangerschaftsdermatose im Detail. Der intrahepatischen Schwangerschaftscholestase ist ein eigener Beitrag gewidmet (Kremer A, Ständer S, DOI 10.1007/s00105-016-3923-y). Des Weiteren wird ein hilfreicher Algorithmus zu Diagnostik und Management von Pruritus in der Schwangerschaft präsentiert.

No correlation between two-dimensional measurements and three-dimensional configuration of the pharyngeal upper airway space in cone-beam computed tomography

The aim of this study was to evaluate both the 2-dimensional (2D) and 3-dimensional (3D) configuration of the pharyngeal airway space (PAS) in healthy 239 patients using cone-beam computed tomography (CBCT). In particular, we investigated the correlation between the 2D and 3D measurements in view of the potential impact on diagnosis of upper airway obstruction.The pharyngeal airway of each patient was studied at five levels: the level of the posterior nasal spine (PNS), the level of half-height of the PNS and the tip of the soft palate, the level of the tip of the soft palate, the level of the anterior-inferior point of the second vertebra, and at least the level of the top of the epiglottis. At each of these levels, the anterior-posterior dimension as well as the corresponding cross-sectional area was measured. Furthermore we measured the volume of the whole PAS between the first and the last level and compared it with the corresponding 2D values.Differences between genders and age effects were estimated in an analysis of covariance. P values <0.05 were considered significant. Linear models were fitted to find the optimal formula to calculate the area or volume from measured distances. Limits of agreement were calculated as 2 ± residual standard deviations. We generated normal 2D and 3D values of the PAS for a normal population. In regard to age, no significant differences were observed. Significant differences were assessed between healthy males and females; however no correlation was observed between the obtained measurements in 2D axial view and the corresponding cross-sectional areas and mean volume in 3D view. In summary, radiologic imaging is a helpful tool to evaluate the airway space in patients with OSAS. However, drawing conclusions from 2D PAS imaging to assess 3D PAS configuration is not reliable, since there is no sufficient correlation between posterior-anterior distances and the corresponding cross-sectional areas.

Role of microbiological culture and polymerase chain reaction (PCR) of Actinomyces in Medication-related Osteonecrosis of the jaw (MRONJ)

We hypothesized that local infection plays a critical role in the pathogenesis of medication-related osteonecrosis of the jaw (MRONJ). Recent developments in molecular methods have revolutionized new approaches for the rapid detection of microorganisms including those difficult to culture. The aim of our study is to identify the bacterial profiles in MRONJ by microbiological culture and polymerase chain reactions (PCR). A retrospective analysis was performed on MRONJ patients from 2008 to 2014. The bacterial profile from MRONJ bone samples was determined using microbiological culture and PCR. Ninety five patients fulfilled the inclusion criteria with mean age of 69.85 ± 8.71 years. A female predilection was detected. The mandible was more commonly affected than maxilla. Tooth extraction was the frequent triggering factor. Breast cancer was the primary cause for administration and intravenous bisphosphonates were the most commonly administrated antiresorptive drugs. The majority of patients were classified as stage 2. Posterior teeth were most commonly affected. Based on bone culture results, the most common microorganism were both actinomyces and mixed flora. PCR confirmed the presence of actinomyces in 55 patients. Our data suggest that PCR might be an innovative method for detection of microorganisms difficult to culture using traditional microbiological techniques.

Orbital exenteration (OE) is considered to be a mutilating surgical procedure reserved for relentlessly progressive neoplastic disorders or extensive facial trauma with unfavourable eye involvement. Malignant tumours, accounting for the majority of ablative orbital surgeries, may be caused by primary orbital tumours or secondarily by neoplasias from the surrounding skin, the maxillary sinus or intracranial malignomas. Orbital exenteration following trauma is mostly caused by penetrating globe defects or extended infections with the danger of intracranial effects. Thoughtful resection planning, the exploitation of reconstructive possibilities as well as the consideration of adjuvant therapy are essential to provide the patient with the best available treatment. As a multitude of reconstructive procedures exist, it is of crucial importance to offer a disease-tailored treatment to achieve a successful patient outcome.After retrospective analysis of 45 orbital exenteration cases within the last decade, we developed a defect-driven classification for ablative orbital therapy followed by a guideline for reconstructive procedures. The classification as well as the reconstruction guideline will help the surgeon to restore anatomic boundaries and to promote physiological and psychological recovery for the patient.

Reliability of the nociceptive blink reflex evoked by electrical stimulation of the trigeminal nerve in humans

Abstract

Objective

The nociceptive blink reflex (nBR) can be useful to investigate trigeminal nociceptive function. The aim of this study was to estimate the reliability of the nBR evoked by electrical stimulation of the three branches of the trigeminal nerve under the following conditions: over time (test-retest and intrarater reliability) and by two examiners (interrater reliability).

Materials and methods

Twenty-one healthy participants were evaluated in two sessions (24 h apart). The nBR was elicited by a so-called "nociceptive-specific" electrode placed over the entry zone of the right supraorbital (V1R), infraorbital (V2R), mental (V3R), and left infraorbital (V2L) nerve. The outcomes were individual electrical sensory (I 0) and pain thresholds (I P); root mean square (RMS), area-under-the-curve (AUC), and onset latencies of R2 responses (determined twice after a recalibration session); and stimulus-evoked pain on a 0–10 numerical rating scale. Intraclass correlation coefficients (ICCs) and Kappa statistics were computed (α = 5%).

Results

ICCs were fair to excellent in 82% of the psychophysical measures (fair 21%, good 31%, excellent 30%) and in 86% of V1R, V2R, and V2L nBR parameters, whereas 52% of V3R showed poor reliability. ICCs for intrarater reliability were fair to good in 70% of measurements (fair 20%, good 50%) and in 75% of interrater measurements after the recalibration (fair 55%, good 20%). All kappa values showed at least fair agreement and the majority of the nBR measures (93%) presented moderate to excellent reliability.

Conclusion

The nBR and its associated psychophysical measures can be considered a sufficiently reliable test.

Clinical significance

The nBR can be recommended as an electrophysiological technique to assess trigeminal nociceptive function.

Microscopy of the lesions revealed hyperkeratotic surface squamous epithelium with papillomatosis and acanthosis along with elongated rete processes. A genomic polymerase chain reaction direct sequencing using the patient's blood was positive for mutations of the PTEN gene typical of CS.

Myoepithelioma of the Nasal Septum: A Rare Case of Extrasalivary Gland Involvement

Introduction. The myoepithelioma is a rare benign tumor, most frequently found in the salivary glands. The extrasalivary gland involvement is even rarer and few cases involving the nasal cavity have been reported in the literature. Case Report. MES, a 54-year-old woman, complaining of progressive nasal obstruction and mild epistaxis through the right nostril which had developed 1 year previously. Computed tomography scan showed tumor with heterogeneous contrast enhancement occupying the right nasal cavity, moving contralaterally in the nasal septum. Excisional biopsy was performed through endoscopic surgery of the mass that was inserted at the nasal septum. Pathological and immunohistochemical exams concluded myoepithelioma. Discussion. The main symptoms of nasal myoepitheliomas are nasal obstruction and epistaxis. Immunohistochemistry is necessary to confirm the diagnosis, typically positive for cytokeratin and S-100, calponin, smooth muscle actin, myosin, vimentin, glial fibrillary acidic protein (GFAP), and carcinoembryonic antigen. The main marker for myoepithelioma is the S-100 protein. In our case, it was positive for cytokeratin, S-100, calponin, actin smooth muscle, and GFAP. In all cases reported in the literature surgical treatment was performed and the recurrence was associated with incomplete tumor resection. Final Comments. The myoepithelioma is a rare differential diagnosis of nasal tumors and its treatment is the total lesion excision.

Distinct expression profile of stem cell markers, LGR5 and LGR6 , in basaloid skin tumors

Abstract

Mammalian epidermis, which is composed of hair follicles, sebaceous glands, and interfollicular epidermis, is maintained by discrete stem cells. In vivo lineage tracing demonstrated that murine LGR5 cells are mainly responsible for hair follicle regeneration whereas LGR6 cells generate sebaceous glands and interfollicular epidermis. However, little is known about their expression in the human skin tumors. In this study, we investigated the expression profile of LGR5 and LGR6 in a variety of human skin tumors including basaloid tumors with follicular differentiation (94 basal cell carcinomas, 18 trichoepitheliomas, 3 basaloid follicular hamartomas, and 12 pilomatricomas) and tumors with ductal differentiation (7 eccrine poromas, 8 hidradenomas, and 5 spiradenomas). LGR5 expression was highest in basal cell carcinomas (BCCs) followed by trichoepitheliomas (TEs) and basaloid follicular hamartomas. LGR6 had the same expression pattern as LGR5, even though its expression was lower. Interestingly, LGR6 expression was detected in stromal cells around the tumor and papillary mesenchymal bodies of TEs but not in stromal cells of BCCs, suggesting different characteristics of tumor-associated fibroblasts between TEs and BCCs. It was unexpected to find that pilomatricomas exclusively expressed LGR6, and its expression was limited to the basaloid cells. Notably, LGR6-positive cells were observed in sweat gland ductal cells in normal skin. This might explain, in part, the finding that LGR6 expression was relatively higher in basaloid tumors with ductal differentiation than in those with follicular differentiation. In particular, spiradenomas displayed the same distribution pattern of LGR6 as normal sweat glands, suggesting the possibility of LGR6-positive cells as tumor stem cells. In conclusion, we documented the different expression patterns of stem cell markers, LGR5 and LGR6 in various skin tumors. These data may provide important insights to understand the origin and development of basaloid skin tumors.

The role of interventional sialendoscopy and intraductal steroid therapy in patients with recurrent sine causa sialadenitis: a prospective cross-sectional study

Objectives

To verify the role of interventional sialendoscopy and steroidal ductal irrigation in patients with recurrent sialadenitis.

Design

A prospective, cross-sectional pilot study.

Setting

University of Milan.

Participants

Fifty-four patients with sine causa recurrent sialadenitis who underwent interventional sialendoscopy (group A, 36 patients) or interventional sialendoscopy followed by a intraductal steroidal irrigations (group B, 18 patients).

Main outcomes measures

The number of episodes of sialadenitis three and 6 months before and after sialendoscopy, and their severity assessed by means of a 0–10 pain visual analogue scale.

Results

In the population as a whole, a significant post-treatment reduction in the number of episodes of 30.7 ± 5.5 after 3 months and 34.6 ± 10.2 after 6 months (P < 0.001) and a significant reduction in pain visual analogue scale values of 4.7 ± 0.4 after 6 months (P < 0.001) occurred. There was a statistically significant reduction in both parameters at the same time points in both treatment groups (P ≥ 0.001), with no significant between-group difference in pain visual analogue scale values, an albeit non-significant trend in favour of group B in terms of the number of episodes 3 months after therapy that became significant after 6 months (11.0 ± 9.9 versus 20.5 ± 9.5; P = 0.05).

Conclusions

Interventional sialendoscopy is effective for the treatment of recurrent sialadenitis; the addition of intraductal steroidal irrigations seems to increase its value in the medium term. Further studies of larger case series with longer follow-up are needed to establish the possibly primary role of steroid therapy in blocking inflammation.

Inferior Vena Cava and Renal Vein Thrombosis Associated with Thymic Carcinoma

Thymic tumors are rare mediastinal tumors that can present with a wide variety of symptoms. They can cause distant manifestations and are frequently associated with paraneoplastic syndromes. In our case, we describe the evolution of a 68-year-old male whose first manifestation was thrombosis of the inferior vena cava and renal veins. Thrombosis of large abdominal veins is rare, especially without being associated with any other comorbidity or risk factors.

Comparison of drain versus no-drain thyroidectomy: a meta-analysis

Abstract

Despite minimal evidence, thyroid drains are routinely used as a precaution against hematoma in thyroidectomy. We undertook the present meta-analysis to evaluate the patient outcomes associated with post-surgical drainage, and whether it offers any advantage over no drainage in patients undergoing thyroidectomy. Randomized and two-arm studies comparing the efficacy of total or partial thyroidectomy with or without post-surgery drainage, in patients undergoing thyroid surgery were included. Medline, Cochrane, EMBASE, and Google Scholar databases were searched until January 22, 2015, using the terms, "thyroidectomy, subtotal thyroidectomy, total thyroidectomy, drainage, thyroid cancer, and goiter." The primary outcome of interest includes postoperative complications such as infection, hematoma, hemorrhage, hypoparathyroidism, recurrent laryngeal nerve palsy, seroma, and duration of hospital stay. 14 studies comprising of 1927 patients were included in the present analysis. The overall analysis revealed that patients in the drain group were more likely to have a higher postoperative infection rate than no-drain group (pooled OR = 2.94, 95 % CI 1.27–6.85, P = 0.012). Similarly, patients in the drain group had a longer hospital stay in comparison to those in the no-drain group (pooled difference in mean = 1.16, 95 % CI 0.72–1.59, P < 0.001). No statistically significant differences between the groups were found for hematoma, hemorrhage, hypoparathyroidism, recurrent laryngeal nerve palsy, and seroma. In conclusion, the current practice of post-surgical drainage in thyroidectomy did not offer any significant advantage. On the contrary, post-surgical infection rate and duration of hospital stay was higher in patients in the drain group.

A novel myringoplasty technique: the placement of a complementary graft descending from the scutum to support an anterosuperior perforation

Abstract

We describe a novel myringoplasty procedure. We placed a separated fascia graft descending from the scutum, combined with underlay myringoplasty, to support an anterosuperior perforation. We reviewed data from patients who underwent myringoplasty procedures to treat perforations extending into the anterosuperior quadrant of the pars tensa from October 2012 to June 2014. A total of 42 patients who were followed for a minimum of 1 year were enrolled. The same technique was used in all operations. The tympanomeatal flap was elevated from the neck of malleus up to the tip of the lateral process of malleus. The anterior mallear fold was incised to create an opening running from the neck of the malleus to the anterior tympanic spine. A separate temporal fascia graft (complementary graft) was next inserted through the opening and pushed down into the protympanum. The upper part of the fascia graft was placed over the superior bony wall of the canal. Underlay myringoplasty was then performed. The inferior part of the fascia graft was next spread out to cover the lateral surface of the underlying graft. We measured graft take rate and preoperative and postoperative hearing parameters. The graft success rate was 97.7 % (41/42 patients). The preoperative air-bone gap was 22.56 ± 18.12 dB, and the postoperative air-bone gap was 8.4 ± 10.05 dB. This difference was statistically significant (P < 0.001). We believe that this myringoplasty technique is a safe, suitable, and effective for cases with tympanic membrane perforations extending into the anterosuperior quadrant of the pars tensa.
