Tracheostomy techniques, indications and care are extensively covered in the literature. However, little is written about the process of removing the tracheostomy tube. At the Royal Hospital for Sick Children in Glasgow we use a stepwise ward-based protocol for safe tracheostomy decannulation. Our aim therefore was to review all the paediatric tracheostomy decannulations that we attempted over the last 3 years to evaluate our protocol, to determine our success rate and to see whether any modifications to the protocol are required. http://ift.tt/29AqW3d

Decreased Symptoms without Augmented Skin Blood Flow in Subjects with RLS/WED after Vibration Treatment

Alexandros G. SfakianakisστοOtoRhinoLaryngology Now by Alexandros G. SfakianakisΠριν από 23 δευτερόλεπτα
Study Objectives: Vascular disturbances leading to tissue hypoxia have been named as a possible cause for RLS/WED. Vibration to the whole body (WBV) in subjects with RLS/WED results in increased skin blood flow (SBF). The aims of this investigation were to (1) determine if a two-week treatment with WBV will decrease symptoms associated with RLS/WED and, (2) if so, determine if the mechanism for improvement in symptoms is related to an increase in SBF, as measured in flux. Methods: Eleven subjects with RLS/WED underwent 2 weeks of 14-minute intermittent WBV and a 2-week sham trea... περισσότερα »

Correcting for Measurement Error in Time-Varying Covariates in Marginal Structural Models

Alexandros G. SfakianakisστοΟγκολογίαΠριν από 1 ώρα
Unbiased estimation of causal parameters from marginal structural models (MSMs) requires a fundamental assumption of no unmeasured confounding. Unfortunately, the time-varying covariates used to obtain inverse probability weights are often error-prone. Although substantial measurement error in important confounders is known to undermine control of confounders in conventional unweighted regression models, this issue has received comparatively limited attention in the MSM literature. Here we propose a novel application of the simulation-extrapolation (SIMEX) procedure to address mea... περισσότερα »

Obesity and obstructive sleep apnea (OSA) have a reciprocal relationship. Sleep disruptions characteristic of OSA may promote behavioral, metabolic, and/or hormonal changes favoring weight gain and/or difficulty losing weight. The regulation of energy balance (EB), i.e. the relationship between energy intake (EI) and energy expenditure (EE), is complex and multi-factorial, involving food intake, hormonal regulation of hunger/satiety/appetite, and EE via metabolism and physical activity (PA). The current systematic review describes the literature on how OSA affects EB-related param... περισσότερα »

One explanation for this is that social stimuli have a heightened reward value at this age. To date, most evidence for heightened social reward in adolescence is found in the animal literature. Human adolescents show increased activation in fronto-striatal brain regions to rewarding social stimuli, but also to negative social stimuli, suggesting that a... περισσότερα »
