Myelin Oligodendrocyte Glycoprotein induces incomplete tolerance of CD4+ T cells specific for both a myelin and a neuronal self antigen in mice
T-cell polyspecificity, predicting that individual T cells recognize a
continuum of related ligands, implies that multiple antigens can tolerize T
cells specific for a given self-antigen. We previously showed in C57BL/6 mice
that part of the CD4+ T-cell repertoire specific for myelin
oligodendrocyte glycoprotein (MOG) 35–55 also recognizes the neuronal antigen
neurofilament medium (NF-M) 15–35. Such bi-specific CD4+ T cells are
frequent and produce inflammatory cytokines after stimulation. Since T cells
recognizing two self-antigens would be expected to be tolerized more
efficiently, this finding prompted us to study how polyspecificity impacts
tolerance. We found that similar to MOG, NF-M is expressed in the thymus by
medullary thymic epithelial cells, a tolerogenic population. Nevertheless, the
frequency, phenotype, and capacity to transfer experimental encephalomyelitis
(EAE) of MOG35-55-reactive CD4+ T cells were increased in
MOG-deficient but not in NF-M-deficient mice. We found that presentation of
NF-M15-35 by I-Ab on dendritic cells is of short duration,
suggesting unstable MHC class II binding. Consistently, introducing an
MHC-anchoring residue into NF-M15-35 (NF-M15-35T20Y)
increased its immunogenicity, activating a repertoire able to induce EAE. Our
results show that in C57BL/6 mice bi-specific encephalitogenic T cells manage to
escape tolerization due to inefficient exposure to two self-antigens.

Proteomic analysis of docetaxel resistance in human nasopharyngeal carcinoma cells using the two-dimensional gel electrophoresis method.
Docetaxel-based chemotherapy has been
recommended for advanced nasopharyngeal carcinoma (NPC). However, treatment
failure often occurs because of acquired drug resistance. In this study, a
docetaxel-resistant NPC cell line CNE-2R was established with increasing doses
of docetaxel for more than 6 months. Two-dimensional gel electrophoresis and
ESI-Q-TOF-MS were used to compare the differential expression of
docetaxel-resistance-associated proteins between human NPC CNE-2 cells and
docetaxel-resistant CNE-2R cells. As a result, 24 differentially expressed
proteins were identified, including 11 proteins with increased expression and 13
proteins with decreased expression. These proteins function in diverse
biological processes such as metabolism, signal transduction, calcium ion
binding, immune response, proteolysis, and so on. Among these, [alpha]-enolase
(ENO1), significantly upregulated in CNE-2R, was selected for detailed analysis.
Inhibition of ENO1 by shRNA restored CNE-2R cells’ sensitivity to docetaxel.
Moreover, overexpression of ENO1 could facilitate the development of acquired
resistance of docetaxel in CNE-2 cells. Western blot and reverse-transcription
PCR data of clinical samples confirmed that [alpha]-enolase was upregulated in
docetaxel-resistant human NPC tissues. Finding such proteins might improve
interpretation of the molecular mechanisms leading to the acquisition of

Paralleled comparison of vectors for the generation of CAR-T cells.
T-lymphocytes genetically engineered with
the chimeric antigen receptor (CAR-T) have shown great therapeutic potential in
cancer treatment. A variety of preclinical researches and clinical trials of
CAR-T therapy have been carried out to lay the foundation for future clinical
application. In these researches, several gene-transfer methods were used to
deliver CARs or other genes into T-lymphocytes, equipping CAR-modified T cells
with a property of recognizing and attacking antigen-expressing tumor cells in a
major histocompatibility complex-independent manner. Here, we summarize the
gene-transfer vectors commonly used in the generation of CAR-T cell, including
retrovirus vectors, lentivirus vectors, the transposon/transposase system, the
plasmid-based system, and the messenger RNA electroporation system. The
following aspects were compared in parallel: efficiency of gene transfer, the
integration methods in the modified T cells, foreground of scale-up production,
and application and development in clinical trials. These aspects should be
taken into account to generate the optimal CAR-gene vector that may be suitable

Genomic Analysis of Immune Cell Infiltrates Across 11 Tumor Types
Background: Immune infiltration of the tumor microenvironment has been
associated with improved survival for some patients with solid tumors. The
precise makeup and prognostic relevance of immune infiltrates across a broad
spectrum of tumors remain unclear.
Methods: Using mRNA sequencing data from The Cancer Genome Atlas (TCGA) from 11 tumor types representing 3485 tumors, we evaluated lymphocyte and macrophage gene expression by tissue type and by genomic subtypes defined within and across tumor tissue of origin (Cox proportional hazards, Pearson correlation). We investigated clonal diversity of B-cell infiltrates through calculating B-cell receptor (BCR) repertoire sequence diversity. All statistical tests were two-sided.
Results: High expression of T-cell and B-cell signatures predicted improved overall survival across many tumor types including breast, lung, and melanoma (breast CD8_T_Cells hazard ratio [HR] = 0.36, 95% confidence interval [CI] = 0.16 to 0.81, P = .01; lung adenocarcinoma B_Cell_60gene HR = 0.71, 95% CI = 0.58 to 0.87, P = 7.80E-04; melanoma LCK HR = 0.86, 95% CI = 0.79 to 0.94, P = 6.75E-04). Macrophage signatures predicted worse survival in GBM, as did B-cell signatures in renal tumors (Glioblastoma Multiforme [GBM]: macrophages HR = 1.62, 95% CI = 1.17 to 2.26, P = .004; renal: B_Cell_60gene HR = 1.17, 95% CI = 1.04 to 1.32, P = .009). BCR diversity was associated with survival beyond gene segment expression in melanoma (HR = 2.67, 95% CI = 1.32 to 5.40, P = .02) and renal cell carcinoma (HR = 0.36, 95% CI = 0.15 to 0.87, P = .006).
Conclusions: These data support existing studies suggesting that in diverse tissue types, heterogeneous immune infiltrates are present and typically portend an improved prognosis. In some tumor types, BCR diversity was also associated with survival. Quantitative genomic signatures of immune cells warrant further testing as prognostic markers and potential biomarkers of response to cancer immunotherapy.

Results: High expression of T-cell and B-cell signatures predicted improved overall survival across many tumor types including breast, lung, and melanoma (breast CD8_T_Cells hazard ratio [HR] = 0.36, 95% confidence interval [CI] = 0.16 to 0.81, P = .01; lung adenocarcinoma B_Cell_60gene HR = 0.71, 95% CI = 0.58 to 0.87, P = 7.80E-04; melanoma LCK HR = 0.86, 95% CI = 0.79 to 0.94, P = 6.75E-04). Macrophage signatures predicted worse survival in GBM, as did B-cell signatures in renal tumors (Glioblastoma Multiforme [GBM]: macrophages HR = 1.62, 95% CI = 1.17 to 2.26, P = .004; renal: B_Cell_60gene HR = 1.17, 95% CI = 1.04 to 1.32, P = .009). BCR diversity was associated with survival beyond gene segment expression in melanoma (HR = 2.67, 95% CI = 1.32 to 5.40, P = .02) and renal cell carcinoma (HR = 0.36, 95% CI = 0.15 to 0.87, P = .006).
Conclusions: These data support existing studies suggesting that in diverse tissue types, heterogeneous immune infiltrates are present and typically portend an improved prognosis. In some tumor types, BCR diversity was also associated with survival. Quantitative genomic signatures of immune cells warrant further testing as prognostic markers and potential biomarkers of response to cancer immunotherapy.

Orbital Recurrence of Ethmoid Sinus Malignant Meningioma: Case Study and Literature Review.
The authors describe a case of a recurrent World Health Organization grade
III extradural, sinonasal meningioma that presented with epiphoria. Primary
extradural meningiomas are rare, comprising only 1% to 2% of all meningiomas.1
These tumors are diagnosed and graded histologically according to the World
Health Organization criteria.2 Grade III tumors are very malignant with high
recurrence and death rates.3 This case highlights the rarity of sinonasal tract
meningiomas involving the orbit. In addition, it shows the poor prognosis of
such malignant disease and the need for a multidisciplinary approach. Collection
and evaluation of protected patient health information for this report was in
compliance with the Health Insurance Portability and Accountability Act and

Sequential incremental doses of bisacodyl increase the diagnostic accuracy of colonic manometry
Abstract
Background
Colonic manometry is the standard diagnostic modality for evaluating colonic
motility in children. Intraluminal bisacodyl is routinely used to trigger
high-amplitude propagating contractions (HAPCs), a feature of normal colonic
motility. Usually, only a single dose (0.2 mg/kg) is suggested. We
retrospectively explored whether the use of an additional higher (0.4 mg/kg)
dose of bisacodyl increases the yield of colonic manometry.
Methods
In 103 children (median age: 8.8 years, range 3.2–15.7 years) with a
diagnosis of slow transit constipation, colonic motility was recorded for 1 h
before and 1 h after each of two incremental doses of bisacodyl (low, L, dose:
0.2 mg/kg, max 10 mg; high, H, dose: 0.4 mg/kg, max 20 mg) and the
characteristics of HAPCs analyzed.
Key Results
High-amplitude propagating contractions were seen in 85 children. H dose
significantly increased the proportion of patients with fully propagated HAPCs
(H dose: 57/103 [55%], L dose: 27/103 [26%], p < 0.001), paralleling
the significant decrease in the proportion with partially propagated HAPCs (H
dose: 29/103 [28%], L dose: 47/103 [46%], p < 0.01). Mean HAPC
number significantly increased throughout the colon at H compared to L dose
(7.2 ± 5.05 vs 5.6 ± 5.1, p < 0.05). Finally, the
proportion of patients with normal pressure wave morphology of HAPCs
significantly increased with higher dose (H dose: 55/85 [65%], L dose: 27/85
[32%], p < 0.001).
Conclusions & Interferences
An additional higher dose of bisacodyl during colonic manometry improves
colonic neuromuscular function suggesting its use might improve interpretation
and decision making in children with slow transit
constipation.

Objective: We propose a computer-aided method for regional ventilation
analysis and observation of lung diseases in temporally resolved magnetic
resonance imaging (4D MRI). Methods: A shape model-based segmentation and
registration workflow was used to create an atlas-derived reference system in
which regional tissue motion can be quantified and multimodal image data can be
compared regionally. Model-based temporal registration of the lung surfaces in
4D MRI data was compared with the registration of 4D computed tomography (CT)
images. A ventilation analysis was performed on 4D MR images of patients with
lung fibrosis; 4D MR ventilation maps were compared with corresponding
diagnostic 3D CT images of the patients and 4D CT maps of subjects without
impaired lung function (serving as reference). Results: Comparison between the
computed patient-specific 4D MR regional ventilation maps and diagnostic CT
images shows good correlation in conspicuous regions. Comparison to 4D
CT-derived ventilation maps supports the plausibility of the 4D MR maps. Dynamic
MRI-based flow-volume loops and spirograms further visualize the free-breathing
behavior. Conclusions: The proposed methods allow for 4D MR-based regional
analysis of tissue dynamics and ventilation in spontaneous breathing and
comparison of patient data. The proposed atlas-based reference coordinate system
provides an automated manner of annotating and comparing multimodal lung image

Surveillance of Small Renal Masses: A Review on the Role of Imaging With a Focus
on Growth Rate.:
Objective: This study aimed to systematically summarize the current
literature in the field of active surveillance for small renal masses, with the
primary focus being the role of imaging in the primary decision-making and
subsequent follow-up. Materials: A systematic review of the electronic databases
PubMed and Web of Knowledge was performed according to Preferred Reporting Items
for Systematic Reviews and Meta-analysis statement guidelines. Variables were
extracted from the data set and included the following: (1) patient
demographics, (2) tumor characteristics, and (3) study design. Results:
Twenty-one articles studying imaging in active surveillance of small renal
masses were selected. Seventy-two percent (15/21) of studies were retrospective;
19% (4/21) were prospective; and 9% (2/21) studies were bidirectional. Mean age
of patients was 69 years (range, 57-81 years). A total of 1386 patients were in
the study; 59% of patients were men. Mean follow-up was 39 months (range,
18.8-91.5 months). Sixty-seven percent of masses discussed in this review were
followed up using more than one imaging modality; 19% consistently used computed
tomography for follow-up whereas the remaining 14% did not specify what imaging
modality was used. Imaging studies were reviewed by the investigators centrally
in 86% (18/21). In 14% of the studies, only imaging report was reviewed. Biopsy
was performed in 24% of masses. Mean growth rate for all tumors was 0.27 cm/y
(range, 0.06-0.7 cm/y). For studies where growth rate of benign and malignant
masses were differentiated, mean growth rate for benign masses was 0.3 cm/y and
mean growth rate for malignant masses was 0.35 cm/y. Conclusions: Growth rate is
often used as a discriminant in following up a small renal mass in patients
undergoing active surveillance. However, there is great variability in growth
rate and it alone is not an adequate marker for determining whether the tumor is
malignant. Because very few studies specified radiological characteristics of
small renal masses, future studies can be done to better characterize masses.

Purpose: The purpose of this study was to evaluate the accuracy of
multidetector computed tomography (MDCT) in the preoperative assessment of renal
sinus fat invasion (RSFI) in patients with renal cell carcinoma (RCC) and to
assess imaging features that improve detection of RSFI on CT. Methods: This is a
single-institution retrospective review of 53 consecutive patients with
histologically proven RCC who underwent triple-phase preoperative contrast MDCT
prior to partial or radical nephrectomy. Two experienced radiologists (R1 and
R2), blinded to the final histology result, independently reviewed the
preoperative MDCT studies to assess for RSFI. Histopathology was used as the
gold standard for the presence of RSFI. Results: Of 55 surgically resected RCCs
that were evaluated with contrast-enhanced MDCT, 34.5% (19/55) of RCCs had RSFI
on final histopathology. Multidetector CT demonstrated high sensitivity (R1,
100%; R2, 93.7%) for the detection of RSFI, but a low positive predictive value
(R1, 40%; R2, 53%) and specificity (R1, 38.4%; R2, 66.6%). Interreader agreement
for RSFI was moderate ([kappa] = 0.56). Renal tumors were significantly larger
in cases with RSFI (6.3 +/- 3.219 cm) than tumors without RSFI (4.1 +/- 2.9 cm)

We present a case of a 30-year-old woman who, while undergoing infertility
evaluation, was found to have a large cystic cervical mass, shown to be cystic
cervicitis on cervical cone biopsy. Differential diagnosis for benign cystic
cervical lesions includes Nabothian cysts, tunnel cluster, endocervical
hyperplasia, and cervicitis, whereas cystic cervical malignancies include
adenocarcinoma and adenoma malignum. While it is crucial to differentiate benign
from malignant lesions, imaging characteristics often overlap and may not
provide a specific diagnosis. In difficult cases, definitive diagnosis should be
