The objective of the study was to evaluate the modifications of glucose and lipid profiles after a switch from lopinavir/ritonavir (LPV/r) to atazanavir, used as ritonavir-boosted (ATV/r) or un-boosted.… Continue reading Impact of switching from lopinavir/ritonavir to boosted and un-boosted atazanavir on glucose metabolism: the ATAzanavir & GLUcose metabolism (ATAGLU) study

Syphilis detection: evaluation of serological screening and pilot reverse confirmatory assay algorithm in blood donors

orlsfakianakisστοOtoRhinoLaryngology by Sfakianakis G.AlexandrosΠριν από 6 δευτερόλεπτα
Serological assays are still considered the most useful tests in the diagnosis of syphilis. Since no single serological assay is able to provide a satisfactory result, in our laboratory we have evaluated the usefulness of a commercially-available immunoblot to diagnose syphilis infection among blood donors. From October 2012 to June 2013, 4572 blood donors were… Continue reading Syphilis detection: evaluation of serological screening and pilot reverse confirmatory assay algorithm in blood donors

Sitting in a Huple, a patented hemisphere-like tool, permanently stimulates the vestibular system, thus improving the balancing ability of children with movement disorders. The Gezenguz Foundation for Children with Birth Injuries has been successfully applying this tool in therapy. By attaching x-Inertial Measurement Unit, a wireless three-dimensional orientation sensor, to the Huple, it can serve as an input peripheral for simple PC games. Children are thus motivated; they willingly perform balance training as well as participate in the game-like test. This improves the accuracy an... περισσότερα »

Can bias evaluation provide protection against false negative results in QT studies without a positive control using exposure response analysis?

Alexandros G. SfakianakisστοΘεραπείαΠριν από 10 δευτερόλεπτα
Abstract The revised ICH E14 document allows the use of exposure-response analysis to exclude a small QT effect of a drug. If plasma levels exceeding clinically relevant are achieved, a positive control is not required. In cases where this cannot be achieved, there may be a need for metrics to protect against false negative results. The objectives of this study were to create bias in ECG laboratory QT interval measurements and define a metric that can be used to detect bias severe enough to cause false negative results using exposure-response analysis. Data from the IQ-CSRC study,... περισσότερα »

Prolonged esophageal acid exposures induce synaptic downscaling of cortical membrane AMPA receptor subunits in rats

Αλέξανδρος ΣφακιανάκηςστοOtoRhinoLaryngology by Alexandros G.SfakianakisΠριν από 10 δευτερόλεπτα
Abstract Background We recently reported the involvement of AMPA (α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid) receptor subunit upregulation and phosphorylation in the rostral cingulate cortex (rCC) as the underlying mechanism of acute esophageal acid-induced cortical sensitization. Based on these findings, we proposed to investigate whether prolonged esophageal acid exposures in rats exhibit homeostatic synaptic scaling through downregulation of AMPA receptor expression in rCC neurons. We intended to study further whether this compensatory mechanism is impaired when rat... περισσότερα »

Purpose of review: Drug-induced hypersensitivity syndrome (DIHS) is a serious adverse drug reaction with potential morbidity and mortality. 'Flare up' or relapses in DIHS is a rare but important feature. These relapses may occur within days to several weeks, even after discontinuation of the offending drug. In this article, we review the pathogenesis of DIHS, implication of human herpesvirus 6 reactivation, and describe published case reports of patients with relapsing DIHS. Recent findings: Common drugs associated with DIHS include (but are not limited to) antiepileptics such as ca... περισσότερα »

Transforming the learning outcomes of anaesthesiology training into entrustable professional activities: A Delphi study.

BACKGROUND: True competency-based medical education should produce graduates meeting fixed standards of competence. Current postgraduate programmes are usually based on a pre-determined length of time in training making them poorly suited for an individual approach. The concept of entrustable professional activities (EPAs) enables a more flexible, personalised and less time-dependent approach to training programmes. An […]

60 YEARS OF POMC: Biosynthesis, trafficking, and secretion of pro-opiomelanocortin-derived peptides

Pro-opiomelanocortin (POMC) is a prohormone that encodes multiple smaller peptide hormones within its structure. These peptide hormones can be generated by cleavage of POMC at basic residue cleavage sites by prohormone-converting enzymes in the regulated secretory pathway (RSP) of POMC-synthesizing endocrine cells and neurons. The peptides are stored inside the cells in dense-core secretory granules […]

60 YEARS OF POMC: Purification and biological characterisation of melanotrophins and corticotrophins

The remarkable conservation of the primary structures and anatomical location of dogfish α-melanocyte-stimulating hormone (MSH), corticotrophin-like intermediate lobe peptide (CLIP) and adrenocorticotrophic hormone (ACTH) compared with mammals reinforced the tissue-specific processing hypothesis of ACTH peptides in the pituitary gland. The cloning of dogfish pro-opiomelanocortin (POMC) led to the identification of -MSH and simultaneously revealed the […]

60 YEARS OF POMC: Adrenal and extra-adrenal functions of ACTH

The pituitary adrenocorticotropic hormone (ACTH) plays a pivotal role in homeostasis and stress response and is thus the major component of the hypothalamo–pituitary–adrenal axis. After a brief summary of ACTH production from proopiomelanocortin (POMC) and on ACTH receptor properties, the first part of the review covers the role of ACTH in steroidogenesis and steroid secretion. 60 YEARS OF POMC: Lipotropin and beta-endorphin: a perspective

Many important fields of research had a humble origin. In the distant past, A J P Martin’s discovery that amino acids could be separated by paper chromatography and Moore and Stein’s use of columns for quantitative amino acid analysis provided the first steps towards the determination of structure in complex biologically active molecules. They opened […]

60 YEARS OF POMC: POMC: an evolutionary perspective

Proopiomelanocortin (POMC) is a complex precursor that comprises several peptidic hormones, including melanocyte-stimulating hormones (MSHs), adrenocorticotropic hormone (ACTH), and β-endorphin. POMC belongs to the opioid/orphanin gene family, whose precursors include either opioid (YGGF) or the orphanin/nociceptin core sequences (FGGF). This gene family diversified during early tetraploidizations of the vertebrate genome to generate four different precursors: […]
