Peritumoral clefting as a key feature in differentiating basal cell carcinoma from trichoblastoma through in‐vivo reflectance confocal microscopy

Abstract

Adnexal tumors represent one of the most troublesome differential diagnosis when dealing with skin lesions with clinical and dermoscopic criteria suggestive for basal cell carcinoma (BCC). Among these, benign follicular tumors, and in particular trichoblastoma and trichoepithelioma, deserve a special mention as BCC mimickers. These are benign tumors showing differentiation toward the outer follicular sheath. Trichoblastoma represents the most common benign tumor arising in a sebaceous nevus, together with the syringocystadenoma papilliferum; it generally appears on the head and neck as a slow growing pigmented or amelanotic papule and several histopathologic variants have been described.
