An Ethnographic Observational Study to Evaluate and Optimize the Use of Respiratory Acoustic Monitoring in Children Receiving Postoperative Opioid Infusions: [image: image]BACKGROUND: Respiratory depression in children receiving postoperative opioid infusions is a significant risk because of the interindividual variability in analgesic requirement. Detection of respiratory depression (or apnea) in these children may be improved with the introduction of automated acoustic respiratory rate (RR) monitoring. However, early detection of adverse events must be balanced with the risk of...περισσότερα »

Airway obstruction by an unexpected equipment damage

Alexandros G. SfakianakisστοOtoRhinoLaryngology - Alexandros G. SfakianakisΠριν από 3 δευτερόλεπτα
Airway obstruction by an unexpected equipment damage: Intubating introducers are often used to facilitate difficult endotracheal intubation. We report a case in which a Frova catheter was used for insertion of a laser-resistant endotracheal tube and in which a strip of plastic was shaved off from the catheter. The plastic strip formed a convolute mass, causing relevant airway obstruction. Clinicians should be aware of this potentially dangerous complication and should be cautious in using Frova intubating introducers in combination with laser-resistant tubes.

Changes in macrophage function modulated by the lipid environment

Alexandros G. SfakianakisστοOtoRhinoLaryngology - Alexandros G. SfakianakisΠριν από 8 δευτερόλεπτα
Changes in macrophage function modulated by the lipid environment: Macrophages (Ms) play a critical role in the defense against pathogens, orchestrating the inflammatory response during injury and maintaining tissue homeostasis. During these processes, macrophages encounter a variety of environmental conditions that are likely to change their gene expression pattern, which modulates their function. In this study, we found that murine Ms displayed two different subpopulations characterized by differences in morphologies, expression of surface markers and phagocytic capacity under no...περισσότερα »

Heat shock proteins and chronic fatigue in primary Sjögrens syndrome

Alexandros G. SfakianakisστοOtoRhinoLaryngology - Alexandros G. SfakianakisΠριν από 8 δευτερόλεπτα
Heat shock proteins and chronic fatigue in primary Sjögrens syndrome: Fatigue occurs frequently in patients with cancer, neurological diseases and chronic inflammatory diseases, but the biological mechanisms that lead to and regulate fatigue are largely unknown. When the innate immune system is activated, heat shock proteins (HSPs) are produced to protect cells. Some extracellular HSPs appear to recognize cellular targets in the brain, and we hypothesize that fatigue may be generated by specific HSPs signalling through neuronal or glial cells in the central nervous system. From a ... περισσότερα »

Delivery of RANKL-Binding Peptide OP3-4 Promotes BMP-2-Induced Maxillary Bone Regeneration: Although bone morphogenetic protein 2 (BMP-2) is known to stimulate osteogenesis, there is evidence that high doses of BMP-2 can lead to side effects, including inflammation and carcinogenesis. The supplementation of other bone-augmenting agents is considered helpful in preventing such side effects by reducing the amount of BMP-2 required to obtain a sufficient amount of bone. We recently showed that a receptor activator of nuclear factor B ligand (RANKL)–binding peptide promotes osteoblast ... περισσότερα »

: A Rare Entity: AbstractThe solitary neurofibroma is a benign tumor of neuronal origin uncommonly reported in the oral cavity. The possible association of this neoplasia with systemic pathologies, such as von Recklinghausen’s disease and multiple endocrine neoplasia, makes its diagnosis fundamental. Solitary isolated neurofibroma of the soft palate not associated with von Recklinghausen’s disease is an unusual tumor and probably only four cases have been reported in English literature previously. To the best of our knowledge, our case of soft palate neurofibroma in 17-year-old mal...περισσότερα »

Our visual abilities are unsurpassed because of a sophisticated code for objects located in the inferior temporal (IT) cortex. This code has remained a mystery because IT neurons show extremely diverse shape selectivity with no apparent organizing principle. Here, we show that there is an intrinsic component to selectivity in IT neurons. We tested IT neurons on distinct shapes and their parametric variations and asked whether neurons selective along one dimension were also selective along others. Selective neurons responded to fewer shapes and were narrowly tuned to local variatio...περισσότερα »

Because of their unique developmental origins and predicted functions, the… from #AlexandrosSfakianakis via Alexandros G.Sfakianakis […]

Diffusion MRI in detecting neuronal activity Neuroscience

Functional MRI (fMRI) is widely used to study brain function in the neurosciences. Unfortunately, conventional fMRI only indirectly assesses neuronal activity via hemodynamic coupling. Diffusion fMRI was proposed as a more direct and accurate fMRI method to detect neuronal activity, yet confirmative findings have proven difficult to obtain. Given that… from #AlexandrosSfakianakis via Alexandros G.Sfakianakis […]

Mutant huntingtin in BG disrupts motor sequences Neuroscience

The basal ganglia (BG) promote complex sequential movements by helping to select elementary motor gestures appropriate to a given behavioral context. Indeed, Huntington’s disease (HD), which causes striatal atrophy in the BG, is characterized by hyperkinesia and chorea. How striatal cell loss alters activity in the BG and downstream motor… from #AlexandrosSfakianakis via Alexandros G.Sfakianakis […]

ppGpp negatively impacts ribosome assembly Microbiology

The stringent response is a survival mechanism used by bacteria to deal with stress. It is coordinated by the nucleotides guanosine tetraphosphate and pentaphosphate [(p)ppGpp], which interact with target proteins to promote bacterial survival. Although this response has been well characterized in proteobacteria, very little is known about the effectors… from #AlexandrosSfakianakis via Alexandros G.Sfakianakis […]

Perceptual learning and prediction error Psychological and Cognitive Sciences

Human perception is shaped by past experience on multiple timescales. Sudden and dramatic changes in perception occur when prior knowledge or expectations match stimulus content. These immediate effects contrast with the longer-term, more gradual improvements that are characteristic of perceptual learning. Despite extensive investigation of these two experience-dependent phenomena, there… from #AlexandrosSfakianakis via Alexandros G.Sfakianakis […]

Protection from Muscle Damage in the Absence of Changes in Muscle Mechanical Behavior.

Introduction: The repeated bout effect characterises the protective adaptation following a single bout of unaccustomed eccentric exercise that induces muscle damage. Sarcomerogenesis and increased tendon compliance have been suggested as potential mechanisms for the repeated bout effect by preventing muscle fascicles from being stretched onto the descending limb of the length-tension curve (the region where sarcomere damage is thought to occur). In this study, evidence was sought for three possible mechanical changes that would support either the sarcomerogenesis or increased tendon ... περισσότερα »

Ultrasound Guidance for Technically Challenging Intrathecal Baclofen Pump Refill: Three Cases and Procedure Description.

Intrathecal baclofen (ITB) therapy is a common treatment used to reduce spasticity due to neurologic disorders and injuries. A variety of factors can increase the difficulty of ITB pump refill. Excess subcutaneous fat overlying the pump, spasticity, suboptimal positioning, pump rotation or inversion, and scar formation over the reservoir fill port can create challenges during pump refill. As a result, multiple unsuccessful attempts at accessing the reservoir fill port can be painful and increase the risk of infection, particularly when repeat skin puncture is required. Such transfer depends on the coexistence of more recruited (source) liquid-like lung regions together with less recruited (target) solid-like lung regions. Pendelluft may improve gas exchange, but because of tidal recruitment, it may also contribute to injury. We hypothesize that higher positive end-expiratory pressur... περισσότερα »
