Differences in clinicopathological characteristics and prognosis between primary and post-irradiation tongue squamous cell carcinoma

BackgroundMany cases of post-irradiation oral carcinoma have been reported, but its prognosis and optimal treatment modalities are not clear. Thus, we compared the clinicopathological characteristics and prognosis between post-irradiation tongue squamous cell carcinoma and primary tongue squamous cell carcinoma.Patients and MethodsA retrospective cohort study was conducted to assess patients with post-irradiation tongue squamous cell carcinoma or primary tongue squamous cell carcinoma. The primary predictor variable was tumor type. The primary outcome variable was prognosis, and the additional outcome variables were clinicopathological characteristic. Survival analysis was performed, and Kaplan–Meier analysis was also conducted to compare the effects of clinicopathological factors on survival using SPSS 18.0 software. Moreover, the Cox model was applied in multivariate analysis.ResultsThe sample was composed of 449 patients grouped as follows: post-irradiation tongue squamous cell carcinoma (n = 68, 47 men and 21 women; age range, 31 to 72 yr) and primary tongue squamous cell carcinoma (n = 381, 247 men and 134 women; age range, 20 to 86 yr). The post-irradiation tongue squamous cell carcinoma patients were found to have a significantly poorer prognosis and several specific clinicopathological characteristics compared with the primary tongue squamous cell carcinoma patients.ConclusionThe post-irradiation tongue squamous cell carcinoma patients had several unique tumor biological characteristics and a poorer prognosis than the primary tongue squamous cell carcinoma patients. Thus, different surgical strategies should be used to treat post-irradiation tongue squamous cell carcinoma patients.
