ObjectivesTo determine the absolute and relative safety of treatment with two concentrations of topical clonazepam solution (0.1 mg/mL, 0.5 mg/mL) for management of oral dysesthesia.Study designA retrospective chart review of patients diagnosed with oral dysesthesia and managed with topical clonazepam solution (swish and spit) between 2008 and 2015. The relative safety of the two concentrations was evaluated in terms of occurrence of adverse drug reactions (ADRs) and occurrence of change to treatment plan secondary to ADRs.Results162 subjects were included, 84 patients in the 0.1 mg/mL cohort and 78 in the 0.5 mg/mL cohort, evaluated at a median follow-up of 6 weeks. Thirty-eight (23%) patients developed ADRs. The most frequently reported ADR was sedation (62% of ADRs), followed by altered mental status and dizziness (7% each). Dose adjustments were required in nine patients (6%), and treatment discontinuation in thirteen (8%). ADRs were more frequently reported in the 0.5 mg/mL cohort, but no significant difference was found in terms of occurrence of ADRs, change to treatment plan secondary to ADRs, or types of ADRs (p>0.05).ConclusionTreatment with topical clonazepam solution in either 0.5 mg/mL or 0.1 mg/mL appears to be safe and well-tolerated. Future prospective studies are needed.

Extramammary paget's disease (EMPD) is an unusual intraepidermal adenocarcinoma, with only two cases in the oral mucosa reported previously. Here, we first report a rare case of Paget's disease of the oral mucosa and perioral skin with extensive contiguous erythematous and erosive manifestations. Ultimately, the patient was treated with oral thalidomide for 8 weeks. We noticed an improvement with resolution of the symptoms. A 3-year follow-up showed no relapse. Thus, this case might provide new insight for clinicians in treating EMPD. More importantly, we advise that dentists should be vigilant for the possibility of some clinically significant conditions in patients with extensive non-specific clinical lesions mimicking inflammatory or infectious diseases in the oral and maxillofacial regions, especially those who are refractory to long-term treatment. Requesting consultations with relevant clinical departments promptly and obtaining histopathological confirmation at an early stage are crucial for early therapy and a better prognosis.

ObjectiveTo evaluate two types of collagen scaffold for gingival regeneration.Study designTwo types of collagen scaffolds, CS-pH7.4 and CS-pH3.0, were prepared by processing atelocollagen at pH 7.4 or 3.0, respectively, followed by dehydrothermal treatment. Gingival wounds with sizes of 4 × 6 mm (rectangle) or 6 mm diameter (circle) were made by buccal incisions in beagle dogs. The defective area was surgically covered with the CS-pH7.4, CS-pH3.0, or no scaffold (control). Gingival regeneration was assessed by monitoring the differences in the lengths of the epithelial and submucosal tissues at the wound site and -normal site. Histopathological assessments were performed by four evaluators independently; statistical significance was evaluated by the Wald test.ResultsSignificantly higher recovery of epithelial and submucosal tissues, resulting in recovery of gum thickness, was observed in gingival wounds treated with the CS-pH7.4 than that in the control. CS-pH3.0 treatment also resulted in higher gingival regeneration than the control; however, the effects were more pronounced in wounds treated with the CS-pH7.4. CS-pH7.4-treated wounds showed better gingival regeneration than control and CS-pH3.0-treated wounds, even after adjusting for interevaluator differences using a linear mixed model.ConclusionsCS-pH7.4 was a promising scaffold for gingival tissue regeneration.

ObjectivesTo compare the performance of CBCT images and digital radiography in the detection of artificial recurrent caries-like lesions under amalgam and composite fillings.Study DesignClass II cavities in 30 molars were filled with amalgam. Fifteen of those molars had the restoration/enamel interface artificially demineralized. Phantoms were prepared. CBCT images were acquired with two units in three voxel sizes (K9000, 0.076 mm; i-CAT, 0.2 mm and 0.4 mm). Intraoral radiographs were obtained with three systems (Digora, VistaScan and RVG-6100). After that, the amalgam fillings were replaced by composite and new images were obtained. Three examiners assessed all images. Sensitivity, specificity, accuracy and ROC curve were calculated and verified through ANOVA and Tukey test.ResultsThere were no significant differences in sensitivity and specificity when the same restorative material was present or when the restorative materials were compared with the imaging technique as a constant. As for accuracy and ROC curve, there were statistically significant differences when the two materials were compared, and in the amalgam group when the different imaging modalities were compared.ConclusionCBCT performed similarly to intraoral radiography in detecting demineralization under restorations. However, the voxel size and the type of restorative material influenced its performance.
