Diagnostic dilemmas of high grade transformation (Richter's syndrome) of chronic lymphocytic leukaemia: results of the phase II NCRI CHOP-OR clinical trial specialist haemato-pathology central review

Alexandros G. SfakianakisστοHead,Neck,Brain,Cervical Spine and Esophageal Cancers by Alexandros G. SfakianakisΠριν από 11 δευτερόλεπτα
Abstract Aims Richter's syndrome refers to high grade transformation of B-cell chronic lymphocytic leukaemia (CLL), usually to diffuse large B-cell lymphoma (DLBCL), as assessed by strict World Health Organisation (WHO)-defined histological criteria. Although a relatively evidence-poor area, the recommended clinical management of high grade transformation differs considerably from relapsed CLL. The 'CHOP-OR' trial was a single arm, multi-centre, non-randomised phase II NCRI trial in patients with newly diagnosed Richter's syndrome, recruiting from across the UK from April 2011 to... περισσότερα »

Age-associated decrease in GDNF and its cognate receptor GFRα-1 protein expression in human skin

Αλέξανδρος ΣφακιανάκηςστοMedicine by Alexandros G. SfakianakisΠριν από 17 δευτερόλεπτα
Summary Glial cell line-derived neurotrophic factor (GDNF) and its cognate receptor (GFRα-1) are expressed in normal human skin. They are involved in murine hair follicle morphogenesis and cycling control. We hypothesize that ‘GDNF and GFRα-1 protein expression in human skin undergoes age-associated alterations. To test our hypothesis, the expression of these proteins was examined in human skin specimens obtained from 30 healthy individuals representing three age groups: children (5–18 years), adults (19–60 years) and the elderly (61–81 years). Immunofluorescent and light microscopi... περισσότερα »

Update on Auditory Evoked Responses: Value of Chirp Stimuli in ABR/ASSR Measurement: When I first heard about chirps many years ago, none of the clinical ABR systems had chirps as stimuli. As time went on, some of the manufacturers began to introduce chirps. As with any other new development, they were heavily promoted. However, I was not going to start using a new type of stimulus when the existing stimuli - traditional tone bursts and clicks - worked just fine. I kept an open mind, but I looked for evidence. Today, there is plenty of evidence for the use of chirps.

Human CK2 is a heterotetrameric constitutively active serine/threonine protein kinase and is an emerging target in current anti-cancer drug discovery. The kinase is composed of two catalytic CK2α subunits and two regulatory CK2β subunits. In order to establish an assay to identify protein-protein-interaction inhibitors (PPI) of the CK2α/CK2β interface, a bioorthogonal click reaction was used to modify the protein kinase α-subunit with a fluorophore. By expanding the genetic code, the unnatural amino acid para azidophenylalanine (pAzF) could be incorporated into CK2α. Performing th... περισσότερα »

In an effort to overcome current limitations to electrical tuning of quantum cascade lasers, a strategy is proposed which combines heterogeneous quantum cascade laser gain engineering with sampled grating architectures. This approach seeks to not only widen the accessible spectral range for an individual emitter, but also compensate for functional non-uniformity of reflectivity and gain lineshapes. A trial laser with a dual wavelength core is presented which exhibits electroluminescence over a 750 cm−1 range and discrete single mode laser emission over a 700 cm−1 range. Abstract Lateral-inhibitory feedback from horizontal cells (HCs) to photoreceptors involves changes in synaptic cleft pH accompanying light-evoked changes in HC membrane potential. We analyzed HC to cone feedback by studying surround-evoked light responses of cones and by obtaining paired whole cell recordings from cones and HCs in salamander retina. We tested three potential sources for synaptic cleft protons: 1) generation by extracellular carbonic anhydrase (CA), 2) release from acidic synaptic vesicles, and 3) Na+/H+ exchangers (NHEs). Management of patients with metastatic squamous cell skin cancer, refractory to initial therapy with standard chemotherapy and radiation protocols, remains difficult with poor overall prognosis and limited therapeutic options. Recently, promising response rates with nivolumab, a programmed death receptor-1-blocking antibody, in squamous cancer of the head and neck have been demonstrated. Considering the similar histological patterns of squamous cell cancer of the skin and squamous cell cancer of the head and neck, we assumed that nivolumab could also be effective in our patients w... περισσότερα »

Retroperitoneal liposarcoma is a rare tumor. The dimension and weight of liposarcoma are variable; those over 20 kg are called 'giant liposarcoma'. Herein, we report giant retroperitoneal liposarcoma measuring 45 cm in diameter and 25 kg in weight encasing the entire left kidney and adherent to adjacent structures. Postictal Magnetic Resonance Imaging Changes Masquerading as Brain Tumor Progression: A Case Series

Seizures are common among patients with brain tumors. Transient, postictal magnetic resonance imaging abnormalities are a long recognized phenomenon. However, these radiographic changes are not as well studied in the brain tumor population. Moreover, reversible neuroimaging abnormalities following seizure activity may be misinterpreted for tumor progression and could consequently result in unnecessary tumor-directed treatment. Here, we describe two cases of patients with brain tumors who developed peri-ictal pseudoprogression and review the relevant literature. Case Rep Oncol 2016;... περισσότερα »

As this loss is permanent in humans, a cell therapy approach has been suggested to replace damaged cells. It is thus of interest to generate lineage restricted progenitor cells appropriate for cell based therapies. Human long-term self-renewing neuroepithelial stem (lt-NES) cell ... περισσότερα »
