Repeated Intravenous Thrombolysis for Early Recurrent Stroke

Alexandros G. SfakianakisστοHead and Neck DiseasesΠριν από 5 δευτερόλεπτα
Challenging the Exclusion Criterion [Brief Report]: Background and Purpose— Intravenous thrombolysis (IVT) within 4.5 hours from symptom onset improves functional outcome in patients with acute ischemic stroke. Its use in patients with previous stroke within the preceding 3 months is contraindicated because of the assumed higher risk of intracranial hemorrhage. In addition, tissue-type plasminogen activator may itself promote neurotoxicity and blood–brain barrier disruption. However, safety and effectiveness of repeated IVT is essentially unknown in patients with early (<3 months) ... περισσότερα »

Cochlear implant (CI) speech performance is typically evaluated using well-enunciated speech produced at a normal rate by a single talker. CI users often have greater difficulty with variations in speech production encountered in everyday listening. Within a single talker, speaking rate, amplitude, duration, and voice pitch information may be quite variable, depending on the production context. The coarse spectral resolution afforded by the CI limits perception of voice pitch, which is an important cue for speech prosody and for tonal languages such as Mandarin Chinese. The authors report a patient of extensive fibrosis within the tympanic membrane in a 65-year-old woman who did not have a history of ear infection. The patient complained of slow progressing hearing loss on the left side without otorrhea or otalgia. Otoscopic examination showed a thick tympanic membrane with whitish mass-like bulge, and pure-tone audiometry revealed conductive hearing loss. The lesion was totally resected, and final findings of a histopathologic examin... περισσότερα »

Is Mathematics a Domain for Philosophers of Explanation?

Alexandros G. SfakianakisστοΙατρικά Άρθρα by Alexandros G. SfakianakisΠριν από 56 δευτερόλεπτα
Abstract In this paper we discuss three interrelated questions. First: is explanation in mathematics a topic that philosophers of mathematics can legitimately investigate? Second: are the specific aims that philosophers of mathematical explanation set themselves legitimate? Finally: are the models of explanation developed by philosophers of science useful tools for philosophers of mathematical explanation? We argue that the answer to all these questions is positive. Our views are completely opposite to the views that Mark Zelcer has put forward recently. Throughout this paper, we s... περισσότερα »

Abstract Heterotopic implantation of transcatheter tricuspid valve is a new treatment option for tricuspid regurgitation. Transcatheter tricuspid valves are implanted onto the cavoatrial junction in order to avoid the challenging task of anchoring the valve onto the complex tricuspid valve annulus. However, little is known about optimum extent of oversizing of the valved stent in a vena cava. In this study, we implanted valves of the same diameter onto the larger sized inferior vena cava (IVC) and a smaller sized superior vena cava (SVC). The valve in the IVC was oversized by 10.7%... περισσότερα »

Abstract Most existing studies of cardiac arrhythmia rely on surface measurements through optical or electrical mapping techniques. Current density imaging (CDI) is a method which enables us to study current pathways inside the tissue. However, this method entails implementation complexities for beating *ex vivo* hearts. Hence, this work presents an approach to simulate and study the current distributions in different cardiac electrophysiological states. The results are corroborated by experimental data, and they indicate that different states were distinguishable. The CDI simulat... περισσότερα »

[image: scifeed_large.png] PD-1/PD-L1 (programmed death 1/programmed death ligand 1) pathway plays a critical role in immune escape of tumor cells. Recent studies have described that PD-L1 is heterogeneously expressed in various types of cancer, although its prognostic/predictive value is still uncertain. These problems are partly due to a not well defined operating protocol for its detection by immunohistochemistry, but also because most of the studies conducted on large case series were made by Tissue Micro Array (TMA). We are going to discuss this latter point, to highlight that... περισσότερα »

Immune checkpoint inhibitor treatment represents a promising approach toward treating cancer and has been shown to be effective in a subset of melanoma, non–small cell lung cancer (NSCLC), and kidney cancers. Recent studies have suggested that the number of nonsynonymous mutations (NsM) can be used to select melanoma and NSCLC patients most likely to benefit from checkpoint inhibitor treatment. It is hypothesized that a higher burden of NsM generates novel epitopes and gene products, detected by the immune system as foreign. The stability of markers that identify cancer cells that propagate disease is important to the outcomes of targeted therapy strategies. In human melanoma, conflicting data exist as to whether hierarchical expression of CD271/p75/NGFR (nerve growth factor receptor) marks cells with enriched tumorigenicity, which would compel their specific targeting in therapy. Antibody-directed chemotherapy (ADC) offers an advantage over conventional chemotherapy because it provides antibody-directed targeting, with resultant improvement in therapeutic efficacy and reduced toxicity. Despite extensive research, with notable exceptions, broad clinical application of ADC remains elusive; major hurdles include the instability of antibody–chemotherapy linkers and reduced tumor toxicity of the chemotherapy when bound to the antibody. To address these challenges, we have developed a platform technology that utilizes the nab-paclitaxel formulation of paclitaxel, ... περισσότερα »

BACKGROUND AND PURPOSE: Although neuroimaging plays an important role in the diagnosis of idiopathic normal pressure hydrocephalus, its predictive value for response to shunt surgery has not been established. The purpose of the current study was to identify neuroimaging markers that predict the shunt response of idiopathic normal pressure hydrocephalus. MATERIALS AND METHODS: Sixty patients… Continue reading High-Convexity Tightness Predicts the Shunt Response in Idiopathic Normal Pressure Hydrocephalus ADULT BRAIN

Physical Inactivity and Epithelial Ovarian Cancer Risk

orlsfakianakisστοOtoRhinoLaryngology by Sfakianakis G.AlexandrosΠριν από 1 ώρα
Background: Despite a large body of literature evaluating the association between recreational physical activity and epithelial ovarian cancer (EOC) risk, the extant evidence is inconclusive, and little is known about the independent association between recreational physical inactivity and EOC risk. We conducted a pooled analysis of nine studies from the Ovarian Cancer Association Consortium to… Continue reading Physical Inactivity and Epithelial Ovarian Cancer Risk

Common Variants in High-Risk Pancreatic Cancer Patients

orlsfakianakisστοOtoRhinoLaryngology by Sfakianakis G.AlexandrosΠριν από 1 ώρα
Individuals from pancreatic cancer families are at increased risk, not only of pancreatic cancer, but also of melanoma, breast, ovarian, and colon cancers. While some of the increased risk may be due to mutations in high-penetrance genes (i.e., BRCA2, PALB2, ATM, p16/CDKN2A or DNA mismatch repair genes), common genetic variants may also be involved. In… Continue reading Common Variants in High-Risk Pancreatic Cancer Patients

Exercise and Inflammatory Markers in Breast Cancer Survivors

orlsfakianakisστοOtoRhinoLaryngology by Sfakianakis G.AlexandrosΠριν από 1 ώρα
Several sources of evidence indicate that exercise during and after breast cancer could positively modulate the tumor microenvironment. This meta-analysis aimed to determine the effects of exercise training on mediators of inflammation in breast cancer survivors. We searched for randomized controlled trials published from January 1990 to March 2014. An inverse variance method of meta-analysis… Continue reading Exercise and Inflammatory Markers in Breast Cancer Survivors

Early Life Body Fatness, AMH, and Breast Density

orlsfakianakisστοOtoRhinoLaryngology by Sfakianakis G.AlexandrosΠριν από 1 ώρα
Background: Emerging evidence suggests positive associations between serum anti-Müllerian hormone (AMH), a marker of ovarian function, and breast cancer risk. Body size at young ages may influence AMH levels, but few studies have examined this. Also, no studies have examined the relation of AMH levels with breast density, a strong predictor of breast cancer risk.… Continue reading Early Life Body Fatness, AMH, and Breast Density

Cancer Prevention Guideline Adherence and Cancer Outcomes

orlsfakianakisστοOtoRhinoLaryngology by Sfakianakis G.AlexandrosΠριν από 1 ώρα
Many studies have reported that adherence to health promotion guidelines for diet, physical activity, and maintenance of healthy body weight may decrease cancer incidence and mortality. A systematic review was performed to examine associations between adherence to established cancer prevention guidelines for diet and physical activity and overall cancer incidence and mortality. PubMed, Google Scholar,… Continue reading Cancer Prevention Guideline Adherence and Cancer Outcomes

Colorectal Carcinoma in HIV-Positive Patients

orlsfakianakisστοOtoRhinoLaryngology by Sfakianakis G.AlexandrosΠριν από 1 ώρα
Background: Emerging evidence suggests differences in colorectal cancer in HIV-infected patients (HIV+) compared with HIV– patients. Microsatellite instability (MSI), occurring in a subset of colorectal cancer, is present at a higher rate in certain cancers in HIV+ patients. Colorectal cancer with MSI share some characteristics with those reported for HIV+ colorectal cancer. On this premise,… Continue reading Colorectal Carcinoma in HIV-Positive Patients

Prevalence and Comorbidity Burden in Older Survivors

orlsfakianakisστοOtoRhinoLaryngology by Sfakianakis G.AlexandrosΠριν από 1 ώρα
Background: Cancer survivors are a growing population, due in large part to the aging of the baby boomer generation and the related «silver tsunami» facing the U.S. health care system. Understanding the impact of a graying nation on cancer prevalence and comorbidity burden is critical in informing efforts to design and implement quality cancer care… Continue reading Prevalence and Comorbidity Burden in Older Survivors

Correspondence Happiness, health, and mortality – Authors’ reply

orlsfakianakisστοOtoRhinoLaryngology by Sfakianakis G.AlexandrosΠριν από 1 ώρα
Results from our prospective UK Million Women Study1 have shown that happiness itself has no direct effect on mortality (except insofar as unhappiness makes people do unhealthy things, such as smoking). The widespread but mistaken belief that unhappiness and stress directly cause ill health has come partly from studies that confuse cause and effect, as… Continue reading Correspondence Happiness, health, and mortality – Authors’ reply
