Variation in the site of the lateral femoral cutaneous nerve during harvest of a deep circumflex iliac artery free flap

We describe an interesting anatomical variant of the lateral femoral cutaneous nerve that we recently encountered during harvest of a deep circumflex iliac artery (DCIA) free flap from a 64-year-old man. We performed a left hemimaxillectomy to resect a T2N0M0 squamous cell carcinoma of his left maxillary alveolus, and successfully reconstructed the defect. As we raised the flap, we noted an anatomical variant of the site of the lateral femoral cutaneous nerve, which meant that we had to transect it to maintain the continuity of the vascular pedicle.
