Primary intra-osseous adenoid cystic carcinoma(IACC) of the mandible is poorly understood due to its rarity. This study performed a comprehensive literature review on the IACC of the mandible. Forty-five cases of IACC reported in literatures and two additional cases present in our hospital were reviewed. The conclusions were that IACC of the mandible generally occurred in the fourth to sixth decades, with no significant gender predilection. Pain and swelling were the most common clinical manifestations. The diagnosis of IACC should based on the clinical, radiological and pathological examinations. With regards to treatment, radical surgery combined with postsurgical radiotherapy was recommended as the best choice. In addition, the histological subtype of the tumor is an important prognostic factor in patients with IACC.
