A case of Vitamin B12 plus niacin deficiency and literature review

Abstract: Objective To explore the etiology,pathogenesis,clinical manifestation,laboratory examination,electroencephalogram(EEG)characters,therapy and prognosis of niacin deficiency.Methods To analyze the clinical datas by collecting one case diagnosed as niacin deficiency with subacute combined degeneration of spinal cord.Results Niacin deficiency,also called Pellagra,was characterized by "four D syndrome"as main clinical manifestations,namely dermatitis,diarrhea,dementia and death.Pellagra happened frequently between summer and autumn,the etionogy of the disease was multifarious.Treatment with niacin as early as possible may achieve good prognosis.Severe niacin deficiency could induce energy metabolic disturbance,combined with the secondary peripheral and central neurological damage,even subacute combined degeneration of spinal cord.Conclusion Multisystem lesions occur if niacin deficiency persists for a long term.Early diagnosis and treatment can improve the prognosis.
